THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI 53214, is performing market research to determine if there is a sufficient number of qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or (4) Large business firms, to provide a pool / group of Urgent Care (UC) Physicians / Hospitalists, for coverage seven days a week for 2nd and 3rd shifts, for the Oscar G. Johnson Veterans Affairs Medical Center (VAMC), 325 East H Street, Iron Mountain, Michigan 49801, as described in the summarized Statement of Work (SOW). The services will be for a one-year base period, plus four one-year option periods to be exercised at the Governments discretion. Summarized Statement of Work Pool / Group of Qualified UC Physicians / Hospitalist Services for Iron Mountain VAMC: "Pool / Group of Contracted Physicians shall: oProvide coverage in the Iron Mountain VAMC UC / Hospitalist section as well as being available for consultations. oProvide coverage seven days a week, 365 days per year, for 2nd and 3rd shifts. May need to work overtime hours if critical need for Patient Care. oProvide medically necessary UC / Hospitalist services; documentation of assessment and plan of care to meet all medical center policies, performance measures, rules and regulations. oBoard Eligible preferred. oBe certified in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS). This requirement will be awarded on a best value to the Government basis. All interested firms who can meet the requirements stated above should respond, in writing, with: 1.Company name. 2.Address. 3.Point of contact (name//title/telephone number/e-mail address). 4.DUNS number. 5.Tax ID number. 6.Indication of which business category (SDVOSB, VOSB, Small Business or Large Business) for which the organization qualifies. 7.A capability statement that addresses the organizations qualifications and ability to perform as a contractor for the requirement depicted in the SOW. 8.Number of Physicians proposed to meet the requirement. 9.Availability of each physician. Submit responses to the Contract Specialist, Thomas Chesak, at thomas.chesak@va.gov no later than 11:00 a.m. (Central Standard Time), 23 October 2015. Urgent Care / Hospitalist Physician Services for Iron Mountain VAMC shall be referenced on all correspondence regarding this announcement. TELEPHONIC INQUIRIES WILL NOT BE ACCEPTED. The NAICS Code 561320, Temporary Help Services, is applicable to this acquisition; the size standard is $27.5 million. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract. The Government will use responses to this notice to make an appropriate acquisition decision. This is the only notice, no other copies of this notice will be provided.
