Kingsbridge Road Rm. 9b64 Bronx NY 10468 In response to contractor inquiry, a site visit has been established for this requirement. The site visit will be at the Northport VA Medical Center, 79 Middleville Road, Northport, NY 11768, from 10 am-11:30 am Eastern on Monday, October 26, 2015. Contractors interested in the site visit must contact Marshand Boone at marshand.boone@va.gov by Friday, October 23, 2015, at 12 PM Eastern. The due date for capabilities statements as per the format described in this notice has been changed to October 28, 2015. The due dates for additional questions regarding this requirement remains Friday, October 23, 2015. Synopsis: Please note that this is not a request for quotes or proposals, the Government is seeking information for market research purposes only. The Department of Veterans Affairs, Northport VA Medical Center, is conducting a market research to find qualified vendors to provide the following service for the Northport, NY Campus: "The vendor shall remove and dispose of two (2) existing four inch Clayton Relief valves. The vendor shall install two (2) new four inch Clayton Relief valves at the Air Stripper Plant. The vendor shall furnish and install new four inch filler piece and four inch flanged adapter. The vendor shall furnish and install all new bolts, nuts, gaskets, and hardware for complete installation. The vendor shall start-up, configure and test. The North American Industry Classification Systems (NAICS) is 221310. Responses to this notice must be submitting in writing via email and must be received no later than October 28, 2015. The purpose of this notice is to gain knowledge of potential qualified sources for this procurement relative to NAICS 221310. Qualified offerors shall have experience capable of supporting this facility. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities which you have provided services to (a capabilities statement), DUNS number, address, point of contact. If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in System for Award Management (SAM) (see internet site: http://www.sam.gov). Responses to this notice must be submitted in writing (email or fax) and received no later than October 28, 2015 at 3:00PM.
