Description
THIS IS A PRESOLICITATION NOTICE VA258-16-R-0025. The solicitation is intended to be issued on or about 15 December 2015 and will remain open for approximately 30 days. The Veterans Integrated Service Network 18 (VISN 18) has a requirement for contract(s) to provide the labor, materials, transportation, equipment and supervision, in compliance with federal, state, and local regulations to provide Custom Sterile Surgical Packs. Custom packs are made up of individually selected components and are specific to each facility where they are used. The packs are assembled by a manufacturer, sterilized, and shipped to the different locations throughout VISN 18 for storage. It is intended that this competition will be a 100% Service-Disabled Veteran-Owned Small Business (SDVOSB) Set-Aside for vendors who are capable of performing the requirements under the categories described in the solicitation when posted. The business must be registered with and verified by the SAMS website (www.sam.gov) as well as Vetbiz (www.vetbiz.gov) to be eligible for award. The NAICS Code for this requirement is 339112 (Surgical and Medical Instrument Manufacturing). The small business size standard for this NAICS codes is 500 employees. The Federal Supply Code for this requirement is 6515 (Medical and Surgical Instruments, Equipment and Supplies). The Government intends to award multiple Blanket Purchase Agreements (BPAs), but reserves the right to award a single BPA. The contract duration will be as follows: a base period to commence at award and end 30 September 2016, and two (2) one-year option periods. The duration of the base period may vary as it is intended to align the period of performance with fiscal years. The blanket purchase agreement(s) will be utilized by the following VA Health Care Systems within Veterans Integrated Service Network 18 (VISN 18): Phoenix VA Health Care System, 777 E Missouri Ave, Phoenix, AZ 85014 (Station 644); Southern AZ VA Health Care System, 3601 S. 6th Ave, Tucson, AZ 85723(Station 678); New Mexico VA Health Care System, 1501 San Pedro Drive SE, Albuquerque, NM 87108 (Station 501). Interested vendors must be able to successfully meet the requirements for a contract to provide the labor, materials, transportation, equipment and supervision, in compliance with federal, state, and local regulations to provide Custom Sterile Surgical Packs. Custom packs are made up of individually selected components and are specific to each facility where they are used. These packs are subject to change in both quantity and content throughout the duration of the agreement as the needs of the Government change. The packs are assembled by a manufacturer, sterilized, and shipped to the different locations throughout VISN 18 for storage.
