- Attachment III - Past Performance Questionnaire Amendment 000001 - RFP HSTS01-16-R-RES001 (10/21/15) Amendment 000001 - The purpose of this solicitation amendment is to correct mistaken information in the RFP. All changes in the RFP are highlighted in yellow. The attached RFP replaces the RFP issued on 10/16/15 in its entirety. All other terms and conditions of the RFP remain unchanged. The Transportation Security Administration (TSA) is soliciting proposals from Service Disabled Veteran Owned Small Business (SDVOSB) Concerns to perform "Janitorial Services for the TSA at the Ontario International Airport (ONT)" in Ontario, CA. The contractor will provide janitorial maintenance services, seven (7) days a week for approximately 17,844 square feet of TSA-occupied space. The secure and non-secure service areas include: baggage areas, passenger screening checkpoint areas, and office space among others. Contractor employees will be required to meet all TSA and airport security screening requirements which include a criminal history, background and fingerprint checks. Pursuant to FAR 19.501, this acquisition is a SDVOSB set-aside and will be conducted in accordance with FAR Part 12 and Part 15. The North American Industry Classification System (NAICS) code for this acquisition is 561720 and the small business size standard for this NAICS code is $18 million dollars in annual receipts. All responsible sources considered a SDVOSB concern under this NAICS code are eligible to respond to this solicitation. The Transportation Security Administration intends to award a firm fixed price contract with one (1) base year and four (4) one-year option periods, as well as a possible extension of performance not to exceed six months, on a competitive basis to one responsive and responsible Offeror. The Offerors shall be considered responsible as defined within the meaning of FAR 9.104-1. The anticipated award date is on or before November 20, 2015. Potential Offerors are hereby notified that future information about this acquisition, including subsequent amendments, will only be available on the Federal Business Opportunities website www.fbo.gov. Interested parties are responsible for monitoring the FedBizOpps website to assure that they have the most up-todate information about this acquisition. Firms offering a response to this notice should ensure that their registration in the Systems for Award Management (SAM) database http://www.sam.gov is current. Mandatory Site Visit A MANDATORY site visit will be held at Ontario International Airport (ONT) on October 28, 2015 at 8:00 AM Pacific Standard time at Terminal 2 located at 2500 E. Airport Drive, Ontario, CA 91761. Offerors will meet TSA's POC in the lobby of Terminal 2 by the escalator. Coordination shall take place between the Quoter and the TSA staff at ONT. No more than 2 participants from each Offeror will be permitted to attend the site visit. Offerors will contact Mr. Steve Fulton at steve.fulton@tsa.dhs.gov, with a copy to Michael Capovilla at michael.capovilla@tsa.dhs.gov, no later than 12:00 PM local time (Washington, D.C.) on Monday, October 26, 2015 to confirm their attendance at the site visit. Proposals submitted from Offerors who do not attend the site visit shall be rendered as non-compliant and shall therefore be ineligible for award. No technical or contractual questions will be answered nor will any additional instruction outside the Statement of Work be provided at the site visit. Proposal Deadline Proposals shall be submitted electronically to Michael Capovilla at michael.capovilla@tsa.dhs.gov with a copy to the Contracting Officer ( loretta.hawley@tsa.dhs.gov ) no later than 12:00 PM local time (Washington, D.C.) on November 12, 2015. This notice is not considered to be a commitment by the Government nor will the Government pay for information solicited. Request for the solicitation will not be accepted.
