Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-84 issued and effective September 3, 2015. This solicitation VA259-15-Q-0026 is issued as a request for quote (RFQ). The North American Industry Classification System Code (NAICS) for this procurement is 334510 (Electro-medical and Electrotherapeutic Apparatus Manufacturing) using a SBA designated Small Business Size Standard of 500 employees. This RFQ is being issued as a small business set-aside under NAICS Code 334510. Any Award Made as a Result of this Solicitation will be made on an All or Nothing Basis All Items being Solicited (Products and/or Receivables as appropriate) must be of New Manufacture; No Remanufactured or "Grey Market" items allowed. FAR 52.212-4 applies to all Solicitations and Awards. Department of Veterans Affairs, VISN 19, Eastern Colorado Health Care System (ECHCS) Denver VA Hospital is seeking a quote for the items listed below. Solicitation and evaluation of offers are to be IAW FAR 52.211-6: Brand Name or Equal. Minimum desired specifications of the requested item(s) are to be equal to or better than the OLYMPUS HD ULTRA-LIGHT PENDULUM URO CAMERA and listed line items. The new HD Pendulum Camera Head CH-S190-08-LB is at the forefront of technology offering unparalleled low weight and ergonomics with the best image quality in endoscopic surgery ever. In addition, it features NBI for advanced bladder cancer management at the push of a button. 2.At only 65g this ergonomic camera head is very manoeuvrable and comfortable to use. Control is optimised by programmable remote switches, sharpened coupler pins for durable connection, improved focus adjustment and CCD rotation. 3.The unique combination of HD and NBI delivers 20% brighter light compared to prior camera head systems.
