- Form 430-128 Form MMB-135B Form MMB-336B Form 430-132 Drawing 222-D-15209 Form 430-129 Statement of Work Exciter Motor Shaft This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number R15PS01776 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-83. This is a total small business set-aside under NAICS 335312 with an associated small business size standard of 1000 employees. CLIN 0010 Exciter Refurbishment for P1 unit per attached SOW, 1 Job at $_______ for a total of $__________. Total price includes all applicable fees and taxes. FOB: Destination, at Grand Coulee, WA 99133. The provision at 52.212-1, Instructions to Offerors-Commercial applies to this acquisition. Addendum to 52.212-1 substitutes the term "quote" where the term "offer" appears in the provision. Additional Quote Submission Requirements: The contractor must provide evidence of understanding of the requirement through a written technical quote that addresses all of the requirements and shows the clear understanding of the PWS. The contractor must provide a narration that demonstrates Past Performance, either as the prime or major subcontractor, on recent and relevant contracts with same or similar scope within the past three years. The contractor shall provide references for similar services performed as described in the submitted narration, to include contract number (if Federal), and point of contact information. Description of cleaning process including drying temperature limits, drying environmental conditions (booth, wrap, etc.) and chemicals including MSDS sheets. Cleaning process can not damage equipment. Verification the Contractor can meet the critical return date of February 1, 2016. By the provision at 52.212-2, Evaluation-Commercial Items, Technical Capability, Past Performance, and price will be evaluated equally. The provision at 52.212-3, Offeror Representations and Certifications must be completed and be active online at http://www.sam.gov.
