Description

COMBINED SYNOPSIS/SOLICITATION - GENERAL INFORMATION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This procurement shall be conducted under FAR Part 12, Acquisition of Commercial Items, and FAR Part 13, Simplified Acquisition Procedures, and Veteran Affairs Acquisition Regulations (VAAR) as supplemented with additional information in this notice. Award to this solicitation shall be based on the Quoter's ability to provide those specific supplies and services stated in this combined synopsis/solicitation. Pursuant to FAR Part 13.303, Blanket Purchase Agreements (BPA), it is the intent of the Department of Veterans Affairs (VA), Veterans Integrated Service Network (VISN) 22 to establish a Single Award BPA, for the Brand Name or Equal requirement for ThinPrep Pap and Non-GYN reagent kits, and all associated supplies to perform and complete the test, as described within this RFQ solicitation. Solicitation number VA262-16-Q-0060 is issued as a "Brand Name or Equal" request for quotation (RFQ) per the salient characteristics listed within this announcement. The North American Industrial Classification System (NAICS) code for this requirement is 325413 and the selected Federal Supply Code (FSC) is 6640. The Small Business size standard is 500 employees. This RFQ is not set-aside for Small Business and is open to all vendors. Due to time constraints, responses to the solicitation shall be due on Friday, October 30, 2015 at 1500 PST. Quotes must be submitted electronically via email to Gabe Caganap, Contracting Specialist at philip.caganap2@va.gov. Contractors are advised to review and monitor the FBO website for the most up to date information, including all amendments. Correspondence or questions may be directed to Gabe Caganap at philip.caganap2@va.gov, with "VA262-15-Q-1166" in the subject line, within forty-eight (48) hours of the posting of this solicitation.
