Description

Notice of Award: Prospective vendors may submit a notice of interest and capabilities in response to this notice. This notice is not a request for competitive proposals. Information received shall be considered solely for the purpose of determining whether or not to conduct a competitive procurement. Determination by the Government not to compete the proposed contract action upon receipt of any responses shall be within the sole discretion of the Government. Purpose of Contract: Provide health coach training to approximately 25 participants annually. Goals for improved health status that target behavior changes related to overweight/obesity, diabetes prevention, and smoking cessation along with physical activity efforts. Period of Performance: Base Year - 11/01/2015 to 10/31/2016. Option Year One - 11/01/2016 to 10/31/2017.
