Description
The purpose of this amendment is to change the opening and closing dates of this solicitation: The open date will be : November 5, 2015 and Close on November 19, 2015. The Federal Bureau of Prisons, Community Treatment Services Contracting; Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient Substance Abuse and Mental Health Services in Detroit, Michigan. Services should be within a 10 miles radius of the United States Post Office, located at 10721 East Jefferson Avenue, Detroit, MI 48214. Provider should be within ¼ miles of public transportation. Performance periods under this contract will be for a one-year base period to begin on January 01, 2016 with four (4) one-year option renewal periods with service ending (if all options are renewed) on December 31, 2020. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three and Option Year Four, respectively. The Following services are required and may be sub-contracted to another licensed contractor. Base Year: Estimated number of inmates = 60 2000 - Substance Use Intake (one per inmate) for 60 inmates = 60 inmates 2011- Intake Assessments (one per inmate) for 60 inmates = 60 assessments 2010 - Individual Counseling Sessions (30 minutes = 1 unit) 48 units (per inmate) x 60 = 2,880 units 2020 - Group Counseling Sessions (30 minutes = 1 unit) 72 units (per inmate) x 60 = 4,320 units 2030 - Family Counseling Sessions (30 minutes = 1 unit) 36 units (per inmate) x 60 = 2,160 units 5000 - Mental Health Intake (one per inmate) for 10 inmates = 10 inmates 5011 - The request for quote will be available on or after November 05, 2015 and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http: //www.fedbizopps.gov. Hard copies of the request for quote will not be available. The site provides downloading instructions. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The estimated closing date is on or about November 19, 2015. Faith-Based and Community -Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. This requirement is unrestricted. The BOP anticipates awarding a firm-fixed price contract. All responsible business concerns may submit a quote which will be considered by this agency.
