5- W-9 NEEDS TO BE SUBMITTED WITH BID PAPERWORK 4- FAR Clauses 3-Terms & Conditions 2-Vendor Form- MUST BE SUBMITTED WITH BID 1-Scope of Work -Bid Sheet "THIS IS A SUBCONTRACTING OPPORTUNITY" Request for Quote/Proposal 2015-HJCA- Dormitory Door Replacement Deficiency Number: 108163 The Hartford Job Corps Academy (HJCA) located at 100 William Shorty Campbell Street, Hartford, CT., 06106 are desirous of a fully bonded and insured vendor to submit a bid on the federally funded project mentioned herein; and if successfully awarded the subject contract, to satisfactorily perform the following work project according to the established criteria and detailed scope of work contained within same. Potential bidders will be evaluated on references submitted, past history of performance if applicable and submission of a complete bid package; Potential bidders must have or obtain a Bid, Performance, Payment Bond, Dunn & Bradstreet (DUNS) number to be awarded a federal contract and be subject to the Davis Bacon Wage Determination FAR 52.222-6. Potential bidders must also be registered in System Award Management (SAM.GOV) before being awarded a federal contract. Final bids are due to this center, not later than, November 11, 2015 at 4:00 pm. HJCA POC for bid submissions is the Purchasing Department. A mandated walk thru is required for all interested vendors before submitting a bid proposal. A Mandatory contractor walk thru date and time is November 4, 2015 at 10:00 A.M. Eastern. A vendor sign in sheet must be signed by all attending vendors. Any potential bidder must adhere to all facets of the detailed attachment b scope of work. The Purchasing Agent can be reached via email at patricia.uccello@etrky.com Scope of Work 1. Vendors are needed to provide a completed SOW (attachment b) to the Hartford Job Corps Academy. Please complete the Bid Pricing sheet and also submit to the center a detailed outlined proposal on contractor company letterhead identifying all areas of the scope details. See all additional required attachments to be completed as advertised on the FBO.gov website. 2. All bid rate services for the Hartford Job Corps Academy are to be furnished by the successful bidder (s). 3. Please see attached link for obtaining a DUNS number: http://fedgov.dnb.com/webform 4. Please see attached link for registering with the (SAM) https://www.sam.gov/portal/public/SAM/ Submit bids by mail or email to: ETR/Hartford Job Corps Academy 100 William Shorty Campbell Street Hartford, CT 06106 patricia.uccello@etrky.com ATTACHMENT B SCOPE OF WORK 2015-HJCA- Dormitory Doors Replacement Deficiency Number: 108163 Project specifications: Work includes removal and replacement of 51 miscellaneous entrance, bathroom and locker doors in the student dormitory building. Subcontractor will be responsible for the following- Remove all debris and dispose of properly off site Replace 51 wood doors with solid core fire rated doors as specified with attached placards to verify rating. Existing frames to be reused Remove and reuse all hardware including locks, kick plates and closures All doors to be primed sanded and (2) two coats of paint, color to be supplied by Center Provide warranty for workmanship and products and materials used Contractor shall provide its own security for tools or materials used. All work to be performed as to cause least amount of disruption to center operation between the hours of 8:00 AM-3:00 PM Monday - Friday and in accordance to all local and state codes and industry standards. Contractor must abide by Davis Beacon Wage Determinations for workers and site employees and provide certified weekly payroll to HJCA finance and purchasing department. Contractor must adhere to the HJCA Security rules and regulations regarding work hours and center vendor reporting. Subcontractor must provide workers who have successfully completed the OSHA 10 Hour Safety Course and are fully covered by company liability and workers' compensation insurances and the contracting company general liability insurance. Contractor shall complete the project in a timely manner adhering to the contract performance start date and finish agreed by center and contractor. All vendors must completely fill out, sign and return the "2015-HJCA Dormitory Doors Replacement Bid Information Sheet" found on page two of this scope of work with their proposal. 8. All vendors must use the HJCA " Dormitory Doors Replacement Bid Sheet " and submit it electronically to : patricia.uccello@etrky.com All other required items can be submitted by mail or sent scan email electronically. 9. All services are to be bid as written and include all material, clean up and labor costs. 10. All services are to be performed and delivered to our location at 100 William Shorty Campbell Street, Hartford, CT. 06106, by the successful bidder (s). 11. Any and all miscellaneous charges must be included in the price stated on the bid sheet. 12. All orders placed by HJCA will be for all services rendered on this project. 13. S election of the vendor (s) will be based on the overall best value and lowest Fee-For-services pricing from vendors that comply with this scope of work. This is NOT a single source procurement solicitation. 14.
