Description

MTC Terms & Conditions W-9 Form Vendor Self-Certification Form SCA Wage Determination Previous Year Condition Reports Request For Quote This Is A Sub-Contracting Opportunity With Management and Training Corporation (MTC), operator of the Tongue Point Job Corps Center. MTC is requesting proposals for our annual life raft inspection service of four (4) life rafts belonging to two (2) of our training vessels (T/V Columbia, T/V Iuka.) Details of the solicitation are in the attached request for quote (RFQ-2016AnnualLifeRaftService.pdf.) -Please note that there is a specific time frame that this service will need to be completed. We ask that the life rafts are picked-up on December 16, 2015 and returned no later than January 06, 2016. In addition to the bid sheet, all valid bids must include a W-9 and completed MTC vendor self-certification form (attached). Bidders must self-certify under the NAICS listed in the RFQ. (NAICS codes can be found at http://www.census.gov/eos/www/naics/) This solicitation is listed under NAICS code: 488390 Other Support Activities for Water Transportation. All bids are due November 11, 2015 by 5:00 PM (PST)
