- This is a Sources Sought Notice (Request for Information). The Government intends to establish a Blanket Purchase Agreement (BPA) for various surgical supplies compatible with Buffalo Supply Inc. item list for the Department of Surgery at Tripler AMC, Hawaii. Requests for the supplies will be placed against the BPA as required. Period of performance will be for five years. NAICS code is 339112. Attached is the list of items required. Interested parties shall reply to Pacific Regional Contracting Office VIA EMAIL to annette.t.pickering.civ@mail.mil. TELEPHONE REQUESTS WILL NOT BE HONORED. Please indicate your interest and/or ability to provide all the items and also provide a current copy of your price list for all the items and your capabilities statement NO LATER THAN 3 NOVEMBER 2015, 8:00 AM Hawaii Standard Time or 2:00 PM Eastern Time. Quotes will no longer be accepted after the suspense date and time. VENDOR MUST BE ABLE TO PROVIDE 100% of the supplies being requested.
