- Statement of Work - Analytical Testing Request for Quote - Analytical Testing COMBINED SYNOPSIS/ SOLICITATION FOR COMMERCIAL ITEMS (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are requested and a written solicitation will not be issued. (ii) This solicitation/synopsis reference number is N6889816RCBG806 and is being issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-84, Dated 05 October 2015. (iv) This acquisition is being solicited as unrestricted. The NAICS code is 541380 with a size standard of $15,000,000.00. (v) Contractors shall submit a proposal using the provided price sheet for analytical testing of Polychlorinated Biphenyls (PCB) and Toxicity Characteristic Leaching Procedure Metals (TCLP) in accordance with the attached Statement of Work. Laboratories providing environmental testing services to Navy programs shall be accredited for each applicable test method being provided per OPNAV M-5090.1, dated 10 Jan 2014. Laboratories must be certified by the National Environmental Laboratory Accreditation Program (NELAP) or approved equivalent certification IAW OPNAV M-5090.1. Certification must be provided with proposal. Due to the sensitivity and complexity of the samples; responsible sources located within a 20 mile radius of Joint Base Charleston's Weapons Station, Goose Creek, SC may submit a proposal, which shall be considered. (vi) N6889816RCBG806- Laboratories must be capable of performing all required tests within the indicated turn-around time. Testing is required on an as-needed basis. Quantities indicated in Testing Matrix (SOW) are estimates and not all levels of testing will be completed for each sample. (See attached statement of work for details) (vii) The period of performance is one (1) Base year of 10 months plus (4) Option periods of 12 months each. Base Year- 1 Dec 15- 30 Sep 16; 1st Option-1 Oct 16- 30 Sep 17; 2nd Option- 1 Oct 17- 30 Sep 18; 3rd Option- 1 Oct 18- 30 Sep 19; 4th Option- 1 Oct 19- 30 Sep 20. (viii) The following clauses and provisions are incorporated and will remain in full force in any resultant award: (full text of clauses and provisions may be accessed electronically at this website: http://farsite.hill.af.mil). FAR 52.212-1, Instructions to Offerors - Commercial. (ix) FAR 52.212-2, Evaluation - Commercial Items Evaluation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers in the following relative order of importance: 1. Price - The Government will evaluate the total price of the offer for award purposes. 2. Proof of certification from the National Environmental Laboratory Accreditation Program or approved equivalent certification IAW OPNAV M-5090.1. Options: The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). The WAWF routing information will be provided upon award.) • DFARS 252.246-7000, Material Inspection And Receiving Report • AFFARS 5352.201-9101 OMBUDSMAN (APR 2014) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).
