Description
The VISN 4, Veterans Health Administration is issuing this request for information/sources sought notice in order to identify businesses that have the resources, capabilities, and expertise to provide durable medical equipment (DME) services for VISN 4 VAMC's Prosthetic & Sensory Aids Service. Generally, the DME services will consist of providing a comprehensive 24 hour a day 7 day a week (Normal business hours with emergency and weekend capabilities when required.) DME program furnishing all necessary labor, equipment, tools, materials, transportation, training, reports, and facilities necessary for the pick-up, delivery, cleaning, installation, repair (when required) and storage of DME items and also includes providing end user-level education regarding the proper use of the DME item being issued to the patient. AREA OF COVERAGE FOR VISN 4: It is yet to be determined if this will be a single award or multiple award Indefinite Delivery/Indefinite Quantity contract. The contractor shall provide services in part or in whole to VISN 4 VAMC's Prosthetic & Sensory Aids Service in the area(s) of coverage as specified below: State: Delaware Counties: Kent, New Castle, Sussex, State: New Jersey Counties: Atlantic, Burlington, Camden, Cape May, Cumberland, Gloucester, Salem State: New York Counties: Chautauqua, Tioga State: Ohio: Counties: Ashtabula, Athens, Belmont, Monroe, Morgan, Noble, Washington, Jefferson State: Pennsylvania Counties: Adams, Allegheny, Armstrong, Beaver, Bedford, Berks, Blair, Bradford, Bucks, Butler, Cambria, Cameron, Carbon, Centre, Chester, Clarion, Clearfield, Clinton, Columbia, Crawford, Cumberland, Dauphin, Delaware, Elk, Erie, Fayette, Forest, Greene, Huntingdon, Indiana, Jefferson, Juniata, Lackawanna, Lancaster, Lawrence, Lebanon, Lehigh, Luzerne, Lycoming, McKean, Mercer, Mifflin, Montgomery, Montour, Monroe, Northampton, Northumberland, Perry, Philadelphia, Pike, Schuylkill, Snyder, Somerset, Sullivan, Susquehanna, Union, Venango, Warren, Washington, Wayne, Westmoreland, Wyoming, York State: West Virginia Counties: Barbour, Braxton, Brooke, Calhoun, Clay, Doddridge, Garrett, Gilmer, Hancock, Harrison, Jackson, Lewis, Marion, Marshall, Monongalia, Nicholas, Ohio, Pleasants, Preston, Randolph, Ritchie, Roane, Taylor, Tucker, Tyler, Upshur, Webster, Wetzel, Wirt, Wood VISN 4 is seeking responses to the following types of information from businesses that consider themselves to have the resources, capabilities and expertise essential to providing the DME services: 1. Company name, business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), person of contact (including telephone number & email address). 2. Please list States and counties listed above in which the Vendor would be able to provide the listed DME services. 3. Previous experience in providing DME services for government or non-government healthcare entities (i.e., VA Medical Centers, hospitals). 4. Common approach(es) to DME operations. 5. Common obstacles and risks associated with providing DME services. 6. Common performance standards and assessment methods. 7. Other information relevant to DME services that's not addressed in 1- 5 that would be beneficial to VISN 4s endeavor in developing its requirement. Responses to the information requested above may be submitted (1) via email to kevin.balser@va.gov. The results of this market research will assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., small business set-aside, full and open competition, etc.). Responses must be submitted by 4:30 PM (EST) December 1, 2015. This request for information/sources sought synopsis is for planning purposes ONLY.
