Description
This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA249-16-Q-0078 is issued as a Request for Quotation (RFQ). This requirement is Full and Open Competition. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83. The requirement is SB set-aside. The associated NAICS code is 811310 and small business size standard is $7.5 million. The government anticipates awarding a Firm Fixed Price award. Award will be made to lowest price quote which conforms to the requirements within this solicitation. The Memphis VAMC, Engineering Service, located at 1030 Jefferson Avenue, Memphis, TN 38104 needs a contract for base + (4) one-year options to renew for UPS Units Service Maintenance Agreement. The contract period of performance shall start the date of award through 2020. See attached statement of work (SOW) for complete details. The Contractor shall submit a quote on company letterhead and include the following information: name, address, and telephone number and DUNS#. The quote shall have the following information included: quantity, unit frequency, unit price, total cost, applicable shipping charges, offered discounts or proposed delivery listed in the item description as specified above. 2. The Contractor shall complete the annual representations and certifications electronically via the SAM Web site accessed through https://www.acquisition.gov. 3. The Contractor shall provide sufficient evidence that they possess adequate resources, capability, experience, responsibility and integrity to meet the technical capabilities to comply with the requirements of the solicitation. 4. The Contractor shall provide past performance evidence. This assessment is based on the Contractor's record of relevant and recent past performance information that pertains to services outlined in the solicitation requirements. 5. The Contractor shall provide current annual certificates for manufacturer trained personnel. 6. All questions must be submitted by 4pm CST, Monday, November 2, 2015.
