- SOURCES SOUGHT NOTICE for Special Diabetes Program for Indians (SDPI) THIS IS A REQUEST FOR INFORMATION ONLY. This is NOT a solicitation for proposals, proposal abstracts, or quotations. All responses will be used for market analysis in determining the availability of potential qualified businesses that may be used to determine the appropriate acquisition strategy. No solicitation is being issued at this time, but may be issued at a later date. Background: The mission of the Indian Health Service (IHS), Division of Diabetes Treatment and Prevention (DDTP) is to develop, document, and sustain a public health effort to prevent and control diabetes and related chronic diseases in America Indian and Alaska Native (AI/AN) people and communities. The DDTP employs collaborative strategies in the 12 IHS Areas through coordination of a network of Area Diabetes Consultants, and approximately 500 Special Diabetes Program for Indians (SDPI) grant programs. In addition, the DDTP is the primary point of contact for inter-agency as well as intra-agency communication on the prevention and treatment of diabetes and related chronic diseases in AI/AN communities. The IHS provides comprehensive primary health care and disease prevention services to approximately 2.1 million American Indians and Alaska Natives through a network of over 632 hospitals, clinics, and health stations on or near Indian reservations. Facilities are predominantly located in rural primary care settings and are managed by IHS, Tribal, and Urban Indian health programs. The diabetes team provides not only clinical consultation services at Alaska Native Medical Center, but also statewide program technical assistance, epidemiologic data, prevention services, training and education for the IHS Alaska region. Funds are being allocated to the Alaska Area Diabetes Program, for the development of programs directed at improving the care of Alaska Native People with diabetes and at risk for developing diabetes. Funds were also used to enhance the diabetes surveillance statewide registry that had been tracking diabetes in Alaska Native People since 1985. With the addition of grant funds received in 2001, the Community Outreach Program was enhanced to meet the growing demand for services from the rural communities. Funds are also used to support a multidisciplinary team, for the development of patient education materials and to raise community awareness of healthy lifestyles throughout Alaskan communities. Summary of Programs: • Surveillance Program - the gathering of prevalence data on diabetes among Alaska Native and American Indian People living in Alaska as well as tracking of complications. • Technical Assistance - maintain a network of support to the 20+ Alaska Tribal Health System (ATHS) grantees. • Education and Training - annual Alaska Area Diabetes Conference, Advanced Diabetes Health Aide Course, Quarterly Newsletters and Lunch & Learns during field clinics. • Statewide Outreach Activities - including specialty foot and diabetes services. A community outreach program offering: - Opportunities to enhance a community's understanding of diabetes through school programs and community gatherings - on-site consultation for diabetes data and/or programming - professional in-services on current diabetes issues - clinical dietitian / diabetes educator services - physical therapist with expertise in foot care • Other Technical Assistance - including site visits, and online Adobe Connect calls. Comprehensive Program Overview Surveillance Program - Objectives: • Maintain and improve the Alaska Native Diabetes Registry (secondary prevention) • Provide current prevalence and complication data to all Alaska Native diabetes grant programs (secondary prevention) Technical Assistance - Objectives: • Maintain and improve a network of support to the 20+ ATHS grantees • Provide technical assistance on grant application issues Education and Training - Objectives: • Increase provider knowledge of diabetes and its treatment • Increase Community Health Aide/Practitioners' (CHA/P) knowledge of diabetes (secondary prevention) Statewide Outreach Activities - Objectives: • Enhance community's understanding of diabetes through school programs and community gatherings • To prevent the development of diabetes in high-risk individuals in Alaska (primary prevention) • Increase availability of tools for patient education (secondary prevention) • Provide foot care to patients with high risk foot problems • Provide specialized diabetes care including diabetes insulin pump training and continuous glucose monitoring Other Technical Assistance - Objectives: • Assistance with patient listings on the diabetes register • EHR chart reviews to ensure accurate and relevant data for the Annual Audit • RPMS medication formulary and lab package reconciliations, with packages outside DMS to ensure that they are working properly • QMAN, VGEN, PGEN, GEN reports for Web Audit, register cleanup, interim audits reports • Audit logic assistance for diabetes indicators in RPMS EHR • Adobe Connect sessions for training using Visual DMS, DMS, QMAN, and PCC Management Reports Interested firms are requested to provide their aforementioned information, along with interest and capability statements. No pricing input is being requested. The Government requests interested parties submit a written response to this notice which includes: 1. Company name and a brief description of their company's business size (i.e. annual revenues and employee size) 2. Company point of contact, mailing address, telephone and fax numbers, DUNS number, and website address 3. Name, telephone number, and e-mail address of a company point of contact who has the authority and knowledge to clarify responses with Government representatives 4. Business status (i.e., Small Business, SDB, 8(a), HUBZone, SDVOSB, WOB, etc.). Type of company is validated via the System for Award Management (SAM). All respondents must register on SAM located at https://www.sam.gov 5. Description of same/similar services offered to the Government and to commercial customers. Any responses involving teaming agreements should delineate between the work that will be accomplished by the prime and the work accomplished by the teaming partners. 6. Anticipated teaming arrangements (i.e. prime contractor/subcontractor, joint venture, mentor-protege relationship, etc.) 7. Applicable GSA Schedule number or other available procurement vehicle 8. Date Submitted Responses must be submitted not later than Friday, November 13, 2015, 12:00 P.M. (EST). Capability Statements will not be returned and will not be accepted after the due date. The maximum number of pages for submission is three (3) pages (excluding cover page). Capability statements should be e-mailed directly to both Points of Contact (POC) listed below. SUBMISSIONS WILL ONLY BE ACCEPTED VIA ELECTRONIC MAIL. * TELEPHONE CALLS WILL NOT BE ACCEPTED * Disclaimer and Important Notes: This notice does not obligate the Government to award a contract. Any information provided by industry to the Government as a result of this sources sought synopsis is strictly voluntary. Responses will not be returned. No entitlements to payment of direct or indirect costs or charges to the Government will arise as a result of contractor submission of responses or the Government's use of such information or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published on a government GPE. However, responses to this notice will not be considered adequate responses to a solicitation.
