Description
INTRODUCTION This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified sources; (2) whether they are other than small businesses; HUBZone small businesses; service-disabled, veteran-owned small businesses; 8(a) small businesses; veteran-owned small businesses; woman-owned small businesses; or small disadvantaged businesses; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition strategy. BACKGROUND In 1997, Congress established the Special Diabetes Program for Indians (SDPI) as a response to the diabetes epidemic among American Indians and Alaska Natives. The Northwest Tribes recognized a need for technical assistance with diabetes data, and The Western Tribal Diabetes Projects (WTDP) has been established to support the need. WTDP assists tribal programs in tracking and reporting accurate health data. This information is used to improve the quality of patient care, to gain additional resources, and to plan effective intervention programs. To respond effectively to the needs of the Northwest Tribes, the WTDP goals are to: • Provide culturally-appropriate training and technical assistance to I/T/U health care programs using diabetes-specific tools and the Resource and Patient Management System (RPMS) and NextGen. • Continue to refine and provide effective and innovative diabetes project tools. • Build partnerships and collaboration for expansion of resources and services to tribal diabetes programs. • Demonstrate and provide culturally appropriate training to tribal programs on various Best Practices and topics, such as: tobacco cessation, nutrition, physical activity, eye exams, motivational interviewing, Healthy Heart and Diabetes Prevention Program curricula, traditional foods/gardens, and motivational interviewing RESPONSE INFORMATION AMERICAN INDIAN/ALASKA NATIVE, HUB ZONE SMALL BUSINESSES, 8(A) FIRMS AND SMALL BUSINESS FIRMS ARE STRONGLY ENCOURAGED TO SUBMIT CAPABILITY STATEMENTS. THE CAPABILITLY STATEMENT MUST CLEARLY DEMONSTRATE THE FIRM'S CAPABILITY AND EXPERIENCE IN DATA SYSTEM IMPLEMENATION; MANAGEMENT; ENHANCEMENT; ERROR RESOLUTION; AND MAINTENENANCE SERVICES. In order to respond to this notice, interested firms shall address the depth and breadth of in-house professional and technical capabilities, and must be able to clearly convey their experience and/or ability to perform in the following areas, which are identified as core services. It is to be noted that capability statements are not sufficient for effective evaluation of their capacity and capability to perform the work required. The contractor shall address its capability and relevant experience to perform each of the main tasks and deliverables below: • Complete a needs assessment survey to help plan WTDP program activities and prioritize site visits to the Northwest Tribes; • Provide assistance with patient listings on the diabetes register; • Conduct EHR chart reviews to ensure accurate and relevant data for the Annual Audit; • Provide RPMS medication formulation and lab package reconciliations, with packages outside DMS to ensure that they are working properly; • Generate QMAN, VGEN, PGEN, GEN reports for WebAudit, register cleanup, interim audits reports; • Audit logic assistance for diabetes indicators in RPMS HER; • Coordinate AdobeConnect sessions for training using Visual DMS, DMS, QMAN, and PCC Management Reports; • Provide a minimum of four RPMS trainings in the Diabetes Management System (DMS) and other electronic tools for managing patient information; • Prepare and distribute Tribe-specific and aggregate summary reports. Provide additional data analysis and reports at the request of the tribal diabetes programs. Reports include the Health Status Report (HSR), an annual Portland Area Aggregate Report summarizing the Annual Diabetes Care and Outcomes Audit, and the Northwest tribal aggregate report; • Distribute the Diabetes Screening Toolkit to tribes upon request for implementation of community diabetes screening; • Host a series of regional "Best Practices" diabetes trainings in conjunction with the Indian Health Service (IHS) Area Diabetes Consultant; and • Coordinate the Native Fitness Event - a program to enhance physical activity practices in AI/AN SDPI programs. A highly innovative fitness training coordinated in partnership with the, Portland Area Indian Health Service, Nike, and certified trainers from the Native American Fitness Council. NF provides interactive sessions: aerobics, boot camp, sports training and traditional native games. In addition, resource information is disseminated on diabetes interventions, bringing diabetes data to life, promoting physical activity, motivational interviewing, best practices and curriculums, and current diabetes data resources. The goal is for participants to take what they learn home and integrate these fitness activities into programs in tribal communities and Native communities. INSTRUCTIONS TO INDUSTRY Interested small business firms are highly encouraged to respond to this notice. However, firms should understand that generic capability statements are not sufficient for effective evaluation of their capacity and capability to perform the work required. Responses must directly demonstrate the company's capability, experience, and ability to marshal resources to effectively and efficiently perform the objectives described above. The Government requests interested parties submit a written response to this notice which includes: a. Company Name. b. Company DUNS number. c. Company point of contact, mailing address, telephone and fax numbers, and website address d. Name, telephone number, and e-mail address of a company point of contact who has the authority and knowledge to clarify responses with government representatives. e. Date submitted. f. Applicable company GSA Schedule number or other available procurement vehicle. g. Type of Company (i.e., small business, 8(a), woman owned, veteran owned, etc.) as validated via the System for Award Management (SAM). All respondents must register on the SAM located at http://www.sam.gov/. Disclaimer and Important Notes. This notice does not obligate the Government to award contract. Any information provided by industry to the Government as a result of this sources sought synopsis is strictly voluntary. Responses will not be returned. No entitlements to payment of direct or indirect costs or charges to the Government will arise as a result of contractor submission of responses, or the Government's use of such information or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published on a government GPE. However, responses to this notice will not be considered adequate responses to a solicitation. Confidentiality. No proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s). Responses must be submitted via email no later than 12:00pm EST on November 17, 2015. Capability statements will not be returned and will not be accepted after the due date. The maximum number of pages for this submission is five (5) pages.
