- Synopsis: Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. Network Contracting Office (NCO) 3 of the Department of Veterans Affairs is currently conducting a market survey for qualified firms capable of providing "REAGENTS-OPTICAL CALIBRATION KIT" for the New York Harbor VA Healthcare System. The North American Industry Classification Systems (NAICS) is 334516. Responses to this notice must be submitting in writing (email or fax) and must be received not later than November 12, 2015 at 10:00AM. Email: Janice.Brooks@va.gov and Fax: 718-741-4722. No telephone inquiries will be accepted. Interested parties MUST provide company/individual name, a capability statement, examples of same or similar work performed at other facilities, DUNS number and address, point of contact and social-economic. Interested parties who do not submit the above information on or before the response due date and time will not be considered for this procurement. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, women owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 334516. Responses to this notice will be used by the Government to make appropriate acquisition decisions. A solicitation is not currently available. If a solicitation is issued, it will be announced on Federal Business opportunities website http://www.fbo.gov at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. VA New York Harbor Healthcare System Procurement Request for HIV & HCV Testing Supplies Statement of Work Introduction The Veteran Affairs New York Harbor Healthcare System is dedicated to providing the highest quality care to veterans using the abilities of all employees, supported by our commitment to education and research. The VA NYHHS always strives to be the provider of choice of veterans and their families by offering an efficient, integrated quality health care system capable of providing a full range of primary, specialty and chronic health care services in a system that is readily accessible and responsive to change. The VA NYHHS is at the forefront of healthcare for veterans with HIV and HCV. VA's prominence in the field is based on the committed focus of its specialist and long experience with infected patients. The VA has placed a great importance on HIV testing for all veterans regardless of age. Early identification of HIV-positive veterans, HIV viral load monitoring and resistance testing, especially we started HCV viral load and HCV Genotyping, may improve patient outcomes and reduce (HIV and HCV) disease transmission. Laboratory tests are keys at every stage of the HIV and HCV care. All those molecular tests provide clinicians with the information that allow them to manage patients more effectively. Objectives VA NYHHS Path Lab employs the most advance testing systems that enable monitoring progress of HIV and HCV infected patients and offering solid evidence for Physician modification of therapy and drug selection. VA New York Harbor Healthcare System Manhattan Campus is in need of following Medical HIV and HCV Testing Supplies. Vender shall also be responsible that all Federal and State or Local Code requirements are met. Training of Operating Personnel The price quoted shall include contractor responsibility for providing on-site orientation and training (off-site training for HIV and HCV VL). This training shall include actual demonstration, operation and care of the equipment furnished, preventative maintenance and any adjustments or other actions which may be undertaken by operating personnel in the event of failure of equipment, provided that such adjustment or action shall in no way jeopardize the Government's rights under contract guarantee clause. The on-site training for HCV Genotype given by qualified contractor representatives on a date to be determined by the using POC, which shall finish before the end of this year (2015). Place of Delivery and Preventive-Maintenance requirements: Vendor also needs to provide twice preventive maintenance (PM) for m200sp, once for m2000rt and once for ABI sequencer. Vendor shall response emergency service in accordance with the requirements of this contract. The emergency service shall provide daytime, Monday-Friday coverage within 24 hours of notification.
