Statement of work for dormitories Davis Bacon wage requirement Serrato Form 542 for over $30,000 SF 1413 Serrato Form 500 Serrato Form 502 THIS IS A SUBCONTRACTING OPPORTUNITY Project title: Remove existing carpet and replace with new flooring A.General: a. This is a subcontracing opportunity b. Location: Cleveland Job Corps Center 13421 Coit Road Cleveland, Ohio 44110 c. Site visit date: Thursday, November 12, 2015 at 10:00am d. Date of submission: The bids must be received no later than Monday, November 23, 2015 5:00pm central B. Period of Performance : December 17, 2015 thru January 6, 2016 C. Scope of Work: Summary: Two Story Dormitories were built in 2007 with carpet tiles laid in the common areas of both floors. There are only stairs for access to the second level. Students will be off for winter break during the time period allowed for the removal and install of the new flooring. 1. Move and replace furniture as needed to complete project. The furniture consists of area tables, three person sofas, lounge chairs and connected long chairs. The furniture will need to be moved out and replaced for the removal and installation of the flooring 2. Remove, take away and dispose of existing carpet, glue and cove bases. 3. Install the recommended/quality and color equivalent vinyl tile and cove bases. a. F Dorm i. Lobby 72'X70' on first floor ii. TV Lounge 1: 14'X20' on second floor iii. RA Lounge 17'X17' two lounge areas, one on first floor and second on second floor 4. All measurements are approximate. The contractor is responsible for verifying the exact measurements. The contractor will provide all materials required to replace the carpet and cove base removed. 5. Furnish and install transition strips as needed. 6. Contractor will provide all personnel, equipment, tools, materials, supervision and other items and services necessary to complete project. 7. Contractor must provide Shaw Easy Vision 12'X24' vinyl tile Style: 041VF, Collection: Array, Look: Resident Resilient LVT - Loose Lay, Color: Water Chestnut 00543 Roppe cove bases 4" Color: 123 Charcoal or equivalent in quality and product color 8. Work must be completed during December 17, 2015 thru January 6, 2016. D. Requirements a. Applicable licenses b. Certificate of Liability insurance with Serrato Corporation listed as a certificate holder c. W-9 and DUNS# d. Vendor input sheet (see attachments Serrato Form 500 and Serrato Form 502) e. Evaluation Criteria: To be considered for award, the proposal must comply in all material respects with the invitation. Award will be made to that responsible one whose proposal, conforming to the invitation, is most advantageous to Serrato Corporation, considering only price and the price-related factors included in the invitation. Pricing should be provided in accordance with Section E- Pricing Schedule G. Wage Determination (see attachments for requirements)
