Biomed Test Equipment Calibration MAINTENANCE CONTRACT SPECIFICATIONS STATEMENT OF WORK 1. Contract Title. Calibration of Biomedical Engineering Test and Measuring Equipment 2. Background. Biomedical Engineering requires annual traceable performance testing and calibration of the test and measuring equipment used in the maintenance and repair of medical equipment and systems. 3. Scope. This contract will cover the performance testing and calibration of included test and measuring equipment identified below for the VA Medical Center located in Philadelphia, PA. 4. Specific Tasks. Contractor will perform performance testing and necessary calibration of the test and measuring equipment listed in the inventory table below. This service will be performed in the month of November. Calibration and measurement standards used in the testing will be traceable to an international or national standard such as the National Institute of Standards and Technology (NIST). Performance testing and calibration will be performed at the VA Medical Center at a mutually agreeable time and date. Contractor will contact Contracting Officer's Representative (COR)/Biomedical Engineering for scheduling. COR/Biomedical Engineering will provide suitable space and utilities necessary for Contractor to perform this service. COR/Biomedical Engineering will also deliver all test and measuring equipment to be inspected to this space. Contractor will provide an individual calibration certificate for each item of test and measuring equipment which undergoes performance testing and calibration. At a minimum, this certificate will include the following information: a.VA EE number of the equipment being tested. b.Serial number of the equipment being tested. c.Model of the equipment being tested. d.Manufacturer of the equipment being tested. e.Description of the equipment being tested (i.e., oscilloscope, electrical safety analyzer, etc.) f.Testing interval. g.Test date. h.Next test due date. i."As received" and "As returned" status of the equipment being tested (e.g., was equipment in tolerance or not when received for testing and was equipment in tolerance or not when returned). j.Calibration results data. k.Identification of the device used to test equipment. l.Statement of traceability, to include the tolerance range for test equipment, to the national or international standard used in the testing and calibration of the equipment. The national or international standard will be specified on the certificate. The results shall be viewable by the COR/Biomedical Engineering. m.Reference to the testing procedure used. n.Contractor identification information including company name, address, phone number and name of specific representative completing performance testing and calibration service. The performance testing and calibration service is not considered complete until all calibration certificates are received by COR/Biomedical Engineering. Payment will not be made until the service is complete. Should any test or measuring equipment be unable to be tested or calibrated due to its failure or malfunction, it will be so identified to Biomedical Engineering. Repair of the test or measuring equipment is not included in this contract and would generally require an additional purchase order to complete the repair. The following test and measuring equipment is included in this contract: See attached list Should additional test and measuring equipment not listed above be identified for performance testing and calibration after the award of this contract, the Contracting Officer must be contacted for approval before the service is performed on that equipment. 5. Performance Monitoring. Contractor calibration certificates will be reviewed to ensure performance testing and calibrations are completed as required. 6. Security Requirements. No special security requirements are anticipated. 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). There is no Government-Furnished Equipment or Government-Furnished Information anticipated with this contract. 8. Other Pertinent Information or Special Considerations. a. Identification of Possible Follow-on Work. None anticipated. b. Identification of Potential Conflicts of Interest (COI). None anticipated. c. Identification of Non-Disclosure Requirements. None anticipated. d. Packaging, Packing and Shipping Instructions. None anticipated. e. Inspection and Acceptance Criteria. As previously noted under Performance Monitoring. 9. Risk Control. Contractor is responsible for providing all applicable training and protective equipment to protect Contractor's employees from any hazards, including blood borne pathogens, which may be encountered in the performance of this contract. 10. Place of Performance. All work will be performed in Building 2, Room 3A152 at VA Medical Center, 3900 Woodland Avenue, Philadelphia, PA 19104. 11. Period of Performance. Inspections and testing are to be performed in October. 12. Terms and Conditions: a.User Responsibility: The Department of Veterans Affairs will provide the contractor's authorized personnel access to the equipment and adequate working space during performance of any services specified in this agreement. b.Biomedical Engineering Contact Person: The VAMC contact person for this maintenance contract can be reached at (215) 823-4933: i.Primary: Andre Davis, Biomedical Technician ii.Secondary: Lois Alderton, BMET Supervisor The primary, or secondary, contact shall coordinate the scheduling of repair and preventive maintenance visits with the contractor. The contractor's representative shall notify the primary contact, or alternate, either in person (Biomedical Shop Room 3A152, bldg 2) or by telephone (extension 5829) whenever he/she arrives on station. The completed service reports shall be provided to the primary contact, or alternate, upon completion of the work. Failure to contact VA representative or present proper documentation will result in delay of payment. c.Information Security: Equipment, and any related services/support associated with the use of this equipment, must adhere to the Health Insurance Portability and Accountability Act (HIPAA) and all applicable Information Security Requirements of the Department of Veterans Affairs. Hard drives and other storage media contained within the equipment cannot be returned to the company, manufacturer and/or its representative. Hard drives or other storage media shall not be removed from VA property by the contractor. Biomedical Engineering MUST be notified whenever a hard drive or other storage media needs to be removed from the equipment so that the VA can take possession for proper disposal/destruction by the Department of Veterans Affairs. d.Changes to Statement of Work: Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. 13.Confidentiality and Nondisclosure a.It is agreed that: i.The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this task order, are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order. ii.The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order, presented to the contractor, shall be submitted to the CO for response. iii.Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. ?
