Description
Past Performance Questionnaire Wage Determination Quote Form SOW RFQ (1) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (2) The solicitation number is HSBP1015Q0007 and is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-84. The associated North American Industrial Classification System (NAICS) code for this procurement is 811310 with a small business size standard of $7.5 million. (3) This procurement shall be a Total Small Business set-aside. All qualified, responsible sources may submit an offer that shall be considered by the agency. Offerors are advised that they are responsible for obtaining amendments. The resulting contract shall be awarded to the offeror who provides the lowest price technically acceptable. (4) The Department of Homeland Security (DHS), Customs and Border Protection (CBP), Indianapolis, IN intends to award a contract for Firing Range Repair and Maintenance Services. It is required that all contractors submitting a quote review the attached STATEMENT OF WORK to review all requirements. All contractors submitting a quote shall be required to meet the requirements found in the STATEMENT OF WORK. (5) SITE VISIT REQUIREMENTS A site visit and walk through is available before the submission of quotes. This site visit is limited to two key personnel from each organization that will be performing the work on this requirement. The site visit is scheduled for November 18, 2015 at 10:00am Eastern Time. Requests to attend the site visit must be submitted in email to Chance Young at CHANCE.R.YOUNGS@CBP.DHS.GOV (also, cc: Gary Orr via email to: Gary.Orr@cbp.dhs.gov) no later than November 16, 2015 at 10:00am Eastern Time. Notification of attendance shall include the full name of the attendee(s), date of birth and the name of the Company which they represent. Note: Limited public parking is available at Building 77, but Port Authority does provide free bussing for the Public from Federal Circle Station at JFK International Airport. (6) The technical approach shall include: Technical Approach Understanding of the SOW, and Adequate staffing to support the requirements of the SOW. (7) Past Performance A past performance questionnaire has been provided with the RFQ. Each vendor shall attempt to have three (3) responses submitted directly to the Government for relevant projects by the RFQ due date. Relevant projects are those that are similar to those requested in this contract. All references should be either ongoing or completed within the last three years. Vendors must either provide the above information or affirmatively state that it possesses no relevant, directly related, or similar past performance. If no past performance references are submitted, then a rating of neutral will be assigned. (8) The DHS, Customs and Border Protection require all questions and quotes must be submitted to Gary.Orr@cbp.dhs.gov. (9) Note: Period of performance shall be one base year plus four one-year option years.
