Description
**SITE VISIT INFORMATION** An organized site visit has been tentatively scheduled for December 17, 2015 at 10:00 a.m. Participants will meet at the John Day Lock and Dam Project 8th Floor Conference Room. John Day Lock and Dam Project is located at I-84 Exit 109, Rufus, Oregon 97050. Due to security requirements at the powerhouse, you must RSVP AS SOON AS POSSIBLE to attend the site visit. The following information should be submitted to Ryan S. Shoemaker, Contract Specialist, at Ryan.S.Shoemaker2@usace.army.mil : 1. Name of person attending site visit; 2. Company Name; 3. Phone Number; 4. E-mail Address; 5. Date of Birth; 6. Place of Birth; and 7. U.S. Citizen? (Y / N) THE E-MAIL SHALL CONTAIN THE FOLLOWING IN THE SUBJECT LINE: "JOHN DAY BLH TURBINE HUB UPGRADES SITE VISIT". NOTE FOR NON-US CITIZENS: Interested Offerors shall submit photocopies of the Passport and Permanent Resident Card of all Contractor employees who are foreign born and have non-US citizen status. These photocopies shall be submitted at least thirty (30) calendar days in advance of the scheduled site visit to the USACE Security Office. This allows the USACE Security Office sufficient time to run a security check on these affected personnel. The Contractor personnel who fit the above description of non-US citizenship shall not be permitted access to the Project until the Foreign Visitor Authorization is received. The Foreign Visitor Authorization for contractor personnel who are non-US citizens may require up to ninety (90) days for some countries. This will NOT warrant an extension to the proposal due date or to the site visit date. The contractor is expected to provide appropriate representation at the site visit.
