Description

The following is a Sources Sought/Request for Information notice. This posted notice is not a Pre-Solicitation or Solicitation document. The government is not currently soliciting bids, quotes or proposals at this time; however, reserves the right to do so at a later date. The James E. Van Zandt VA Medical Center located at 2907 Pleasant Valley Blvd.; Altoona, Pennsylvania has a future requirement for the provision of Medical Officers of the Day (MODs), and Urgent Care Clinic (UCC) physicians to provide services onsite to eligible VA beneficiaries. The MOD is responsible for the provision of urgent and general medical care to all patients in the UCC, acute care, and long-term care residents in the Community Living Center (CLC) during weeknights, weekends, and Federal holidays. The MOD functions as the primary physician during weeknights, weekends, and Federal holidays. The MOD is responsible for conducting rounds on all inpatients during weekends and Federal Holidays. The UCC Physician is responsible for the provision of urgent care to all patients in the UCC Monday through Friday, excluding Federal holidays for periods of absence of a VA staff physician for scheduled leave. The Contractor will be required to provide qualified physicians to provide all services during the times noted above as the Medical Center does not currently have the staff to provide these required services. Note: All information will be kept confidential and will be used by VA for planning purposes. 1.As a prime or subcontractor have you provided same or similar services? 2.If you have provided same or similar services, where did you provide the services and what was the timeframe of performance? 3.What is the typical profit and overhead percentage used by your company when developing pricing? Please provide answers to all of the below questions regarding your firm's Socio-Economic status pursuant to North American Industrial Classification Code (NAICS) 621111 "Offices of Physicians (except Mental Health Specialists" with a Small Business size threshold of $11.0 million average gross annual receipts. 1.Is your business a small business under NAICS code 621111? 2.Is your firm a Veteran-Owned Small Business? 3.Is your firm a Service-Disabled Veteran-Owned Small Business? 4.Is you firm a certified HUB Zone firm? 5.Is your firm a Woman Owned business? 6.Is your firm a Large Business? 7.Does your firm currently have a contract on the GSA website?
