Description

STATEMENT OF WORK (SOW) Install Small Sterilizer Veterans Healthcare System of the Ozarks (VHSO) 1.SCOPE The contractor shall provide all personnel, labor, tools, materials, supplies, equipment, software, transportation, lodging and supervision necessary to complete all work associated with installation of Steris SR0101110111 AMSCO 400 Series Sterilizer. The installation will be performed on the third floor Building 1 Surgery Department located at the VHSO Campus, 1100 N. College Ave. Fayetteville, AR 72703. The VHSO Medical Center will move the sterilizer from the VHSO Warehouse to the Surgery Department. 2.OBJECTIVES 2.1.The contractor shall convert the sterilizer from the existing electric configuration to direct steam use. This will consist of removing the electric steam generator and adding new piping to allow direct connection to a steam source. 2.2.The contactor shall reprogram the sterilizer for direct steam. The contractor is responsible for having the service code and expertise necessary to reconfigure the internal program. 2.3.The contractor shall provide materials and make final connections to electric, steam, water, and sewer. Electric, water, steam, and sewer hookups will be provided within three feet of the sterilizers location by the VHSO Medical Center prior to installation. 2.4.The contractor shall provide initial startup, testing, and documentation showing that the sterilizer is ready for operation. Any replacement of parts or repairs necessary to get the sterilizer operational shall be the responsibility of the contractor and included in this contract. 2.5.The contractor shall provide a complete 6 month onsite warranty for parts and labor. Response time shall be a minimum of 4 hours callback and 24 hours for technician to arrive onsite. 3.CONTRACTOR QUALIFICATIONS 3.1.All personnel/technicians working on this equipment must have a minimum of three years' experience in the installation of sterilizers used in hospital SPS or Surgery Departments. 3.2.The company must provide documentation for installation and startup of three similar sterilizers manufactured by Steris in the last two years. 4.PERFORMANCE OF WORK 4.1.All work shall be performed on-site at the VHSO Campus. Removal or relocation of equipment is prohibited. 4.2.Performance must take place over a period of one weekday evening. Work must be performed between the hours of 4:00PM and 5:00AM. 4.3.The actual weekday (date) of performance will be identified in the award document. 5.SECURITY REQUIREMENTS 5.1.All contractor personnel shall obtain an access badge from the VHSO Police Department prior to performing work on-site. 5.2.All contractor personnel are required to display the VHSO issued badge at all times while on-site. 5.3.Contractor personnel must contact the COR prior to performing any work in a VHSO facility to determine if a Government escort is required. The COR will arrange for Government escorts if required. 6.SAFETY All contractor personnel shall comply with all federal, state, and local safety laws, regulations, codes and/or standards while performing work associated with this purchase order.
