- PRICING FOR LAB TESTS - RFQ Lab FY16 SOW The United States Property and Fiscal Office for New York (USPFO-NY) proposes to procure Laboratory Pick-up, Transport and Testing for Soldiers participating in annual readiness events conducted throughout New York from 1 December 2015 through 30 November 2018; Base Year Plus 2 Option Years. The contractor is required to provide all material (except that identified as Government Furnished Equipment), labor, supervision and means required to perform the required task per the attached Statement of Work (see "Lab FY16 SOW). This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and are required to be submitted per the enclosed "PRICING FOR LAB TESTS-RFQ" Word document. SPECIFIC TASKS: 1. Contractor will be required to pick up, transport and test specimens in multiple locations across New York State. Common locations listed, but not limited to: a. 1 Buffington Street, BLDG 40-4, Watervliet Arsenal, Watervliet, NY 12189 b. Camp Smith Training Site (CSTS), Cortlandt Manor, NY 10567 c. Fort Drum, Watertown, NY 13602 d. 27 Masten Ave, Buffalo, NY 14204 Dates, locations and estimated quantities are listed in the Performance Work Statement (see "Lab FY16 SOW") 2. Analyze and result specimens with standard of 99.5% tested with no more than 2% error. 3. Record the results and reports to MEDCOM within 48 hours of specimen pick up and testing. Contractor shall be governed and comply with all applicable State and Federal Laws. Contractor will provide for pickup of samples between 0900 to 1800 hours, Monday through Sunday from various sites throughout New York State as designated by NYARNG Medical Command. Once samples are picked up, they will be taken to the contractor laboratory, processed, and tested. The results will be provided to Medical Command in written format within 48 hours. CONTRACTOR FURNISHED ITEMS: Contractor shall possess an owned mobile fleet to accommodate multiple events at the same time. Contracted laboratory will provide all supplies needed to contain, separate, and ship specimens with exception of Government Furnished Equipment. This includes all tubes, sterile urine cups, requisition forms, packing and shipping items, centrifuges, and any other supplies needed to provide safe and secure shipping to the contractor. GOVERNMENT-FURNISHED PROPERTY - Medical Command, NYARNG will provide the required blood drawing supplies (i.e. needles, syringes, band aids, tourniquets) to obtain specimens as needed for laboratory testing. The number of specimens taken will vary daily, as numbers of tests are based on mission requirements. Contractors are directed to check the site often for updates. The Combined Synopsis/Solicitation Number is 0010768776; the due date for quotes is 30 November, 2015 at 10:00 EST. Offerors shall submit their quotes on the attached "PRICING FOR LAB TEST - RFQ" via electronic mail to elizabeth.a.condon2.mil@mail.mil, no later than due date and time. Federal Acquisition Regulation (FAR) provision 52.204-8 Annual Representations and Certification is included in the Combined Synopsis/Solicitation. The Combined Synopsis/Solicitation is set-aside for small business; all responsible parties may submit a quote which will be considered. The total dollar magnitude of this procurement is estimated at between $25,000 and $100,000 per year with a combined total of $100,000 to $500,000 for a 5year period. The North American Industry Classification System (NAICS) Code for this procurement is 541380. The Small Business Size Standard is $15,000,000.
