- Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this requirement is NIH-O D3342016 and this is a Request for Proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-84 dated October 5, 2015. The NAICS code for this requirement is 541611 for $15.0M. The Statement of Work is attached. The Period of Performance will be January 1, 2016 through December 31, 2016 for the base year and four (4) option periods. FAR Pt. 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition and a statement regarding any addenda to the provision. FAR Pt. 52.212-2, Evaluation-Commercial Items is applicable. The specific evaluation criteria: Proposals will be evaluated on : 30% -Past performance in administering this type of program 30%-Key Personnel qualifications and experience in administering similar programs. 25%-Technical and management approach to the implementation of tasks and deliverables stated 15%- Cost/price must be within +/- 10% of fair market value -Past Performance/Relevant Experience- experience in working with the federal government and experience in distributing child care tuition assistance. Offerors shall include a completed copy of the provision at 52.212-3, Offerors Representations and Certifications-Commercial Items, with its offer or submit a printout to their Online Reps and Certs. The clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in SAM at www.sam.gov prior to award, during performance and through final payment of the Purchase Order. Requests for information and (or) questions concerning this requirement are to be submitted by- Tuesday, December 1, 2015, by e-mail ONLY. Offers must be submitted no later than 12:00 P.M. Eastern Daylight Time on Tuesday, December 8, 2015.
