COMBINED SYNOPSIS/SOLICIATION VA246-15-Q-0090 Reception Desk Casework Fabrication and Installation (I)This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Quotes are being requested, and a written solicitation will not be issued. (II)This solicitation is issued as a Request for Quote (RFQ). Submit written quotes on RFQ VA246-15-Q-0090. (III)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83. (IV)Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov). (V)The Network Contracting Office 6-SAO EAST intends to award a firm-fixed price contract for Reception Desk Casework Fabrication and Installation for the Department of Veteran Affairs, Richmond VA Medical Center, 1201 Broad Rock Blvd., Richmond, VA, 23249. (VI)The North American Industrial Classification System (NAICS) code for this procurement is 337110 with a business size standard of 500. This procurement is unrestricted to and for all eligible offerors may submit quotes. All offerors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. Introduction The Hunter Holmes McGuire VA Medical Center in Richmond, Virginia is soliciting bids for fabrication and installation of a new reception desk/ casework to include wall and floor cabinets, solid surface countertops and backsplash, and resin panels for 1E-121 in Building 500 Mental Health Recovery. 2. SCOPE OF WORK 2.1. OVERVIEW Provide all qualified labor, field verification, materials, tools, and supervision for the direct installation of reception desk/ casework to include wall and floor cabinets, solid surface countertops and backsplashes, and reception desk resin panels in Building 500 at the Hunter Holmes McGuire VA Medical Center. Contractor shall field verify all dimensions prior to fabrication and installation. Must follow touchup and repair any damage to adjacent walls, floor and ceiling and return to original condition prior to installation 2.2 SUBMITTALS 1. SUBSTITUTION REQUESTS Substitution requests ("Or Equals") shall meet salient characteristics outlined in Section 2.2.2. Substitution requests shall be similar to the specified dimensions, color scheme, function, and quality. "Or equal" products submissions shall include manufacturer's product brochure including specification information, warranty data, and physical samples of manufacturer's full range of finish options. FAILURE TO CLEARLY NOTE THAT THE PRODUCTS BEING QUOTED ARE THE REQUESTED BRAND NAME, OR FAILURE TO SUPPLY "OR EQUAL" PRODUCTS' SPECIFICATIONS AND INFORMATIONAL LITERATURE SHALL CONSTITUTE A NON-RESPONSIVE BID AND SHALL NOT BE FURTHER EVALUATED FOR AWARD. 2.3 WARRANTY All casework shall be under standard GSA Contract warranties. 2.4 VENDOR FURNISHED ITEMS AND RESPONSIBILITIES The Vendor shall furnish all supplies, equipment, facilities and services required to perform the service under this contract. The Vendor shall be responsible for any missing parts and components not included in order to carry out the installation. The Vendor shall perform all work in accordance with this Performance Work Statement to include the casework and all necessary hardware. Final field verifications shall be the responsibility of the awarded Vendor. Samples shall be submitted for approval and final selection to COR Interior Designer per section 7.Deliverables. Installing vendor shall ensure all staff arrives on site with all necessary tools and equipment required for the installation including dollies, ladders, hand tools, and carts to deliver and install the products as required to installation work site. All vendor staff shall come to the job site equipment with all necessary safety gear including but not limited to eye protection, hard hats, etc. 2.5 DELIVERY AND INSTALLATION 2.5.1. DELIVERY 1.Protect lumber and millwork from dampness, maintaining moisture content specified both during and after delivery at site. 2.Store finishing lumber and millwork in weathertight well ventilated structures or in space in existing buildings designated by Resident Engineer. Store at a minimum temperature of 210C (700F) for not less than 10 days before installation. 3.Pile lumber in stacks in such manner as to provide air circulation around surfaces of each piece. 4.Delivery, Set-up, and Installation shall be coordinated with a RVAMC Interior Designer. INSTALLATION 1.Vendor shall reference Attachments for casework layout per room as defined in Section 2.2. 2.Vendor shall install casework level and true to assure proper operation. 3.The vendor must clean casework and countertop surfaces after installation, according to manufacturer's written instructions. 4.The installation shall be only by qualified and certified installers with the knowledge of commercial casework installations and shall comply with all necessary VA and safety regulations 5.Vendor shall not install casework until spaces are enclosed and weatherproof, wet work in spaces is complete and dry, work above ceilings is complete and ambient temperature and humidity conditions are maintained at the levels indicated for Project when occupied for its intended use. 6.It is preferred that the awarded installation and purchase shall be by a vendor which is local to RVAMC in order to provide service and warranty for repairs as needed within a 24 hours required notice. 3. SITE CONDITIONS 1.The awarded Vendor shall include removal of all related shipping debris and construction cleanup associated with delivery and installation of the specified items. All packaging must be taken off the RVAMC premises by the Vendor. The Vendor shall be responsible for any damage to the building that occurs due to Vendor error or neglect. The Vendor shall be responsible for the coordination of staging areas and parking within the receiving area of the building. All casework deliveries, installations, and arrangements shall be coordinated with the COR Interior Designer. 2.There shall be no smoking, eating, or drinking inside the hospital at any time. Smoking in designated areas outside the buildings shall be allowed. 4. TIME FOR COMPLETION 1.All work must be initiated and completed between November 27, 2015 and January 19, 2016. 2.All work shall be completed between 8:00 a.m. and 4:30 p.m. Monday - Friday. All federal holidays, excluded. 5. EVALUATION OF PROPOSALS Proposals shall be evaluated based on - in order of importance - Past Performance, Price 6. INSPECTION AND ACCEPTANCE 1.The Vendor shall conduct a joint inspection with the Contracting Officer's Representative (COR) once all casework has been installed. The COR Interior Designer shall inspect all phases of delivery and installation with a sign off of any and all missing or damaged products in the form of a vendor provided punch list. Awarded Vendor shall provide detailed punch list items and established dates for replacement parts and/or short ship items from the manufacturer(s). 2.The COR shall ensure all work is completed satisfactorily prior to acceptance. Disputes shall be resolved by the Contracting Officer. 7. DELIVERABLES The reception desk casework shall be installed according to this document and Attachments. Field verification is required prior to procuring items listed, in order to ensure proper installation. Any changes shall be required to be approved by Randalle Burton, Elizabeth Steinburg, or Karen Soucie. Minor adjustments may be required. 1.Installation Schedule: The installation of specified products shall be completed per section 4.Time for Completion, listed above. Successful Vendor shall cooperate with the COR Interior Designer in developing /coordinating an installation schedule, which shall include installation and inspection of the specified item as well as a final walk through with the COR Interior Designer. After examining the site requirements, and conferring with the COR Interior Designer, the successful Vendor shall present an installation schedule and location plan within 10 business days of award. 2.Samples and Literature for Verification: For each type of exposed finish required, provide a sample, not less than three (3) inches square, of same thickness and material indicated for the work. Sample(s) shall be submitted within 10 business days of award. If finishes involve normal color and texture variations, include sample sets showing the full range of variations expected. Literature and specifications shall be submitted within 10 business days of award. 3.Close Out Documentation: a.Two complete sets of documentation, as listed below, shall be provided to the SLVHCS COR Interior Designer at the completion of installation. b.Installation plan c.Punch list 8. PROTECTION OF PROPERTY 1.Vendor shall protect all items from damage. The Vendor shall take precaution against damage to the building(s), grounds and furnishings. The Vendor shall repair or replace any items related to building(s) or grounds damaged accidentally or on purpose due to actions by the vendor. 2.The Vendor shall perform an inspection of the building(s) and grounds with the COR prior to commencing work. To insure that the vendor shall be able to repair or replace any items, components, building(s) or grounds damaged due to negligence and/or actions taken by the Vendor. The source of all repairs beyond simple surface cleaning is the facility construction contractor (or appropriate subcontractor), so that building warranty is maintained. Concurrence from the VA Facilities Management POC and COR is required before the Vendor may perform any significant repair work. In all cases, repairs shall utilize materials of the same quality, size, texture, grade, and color to match adjacent existing work. 3.The Vendor shall be responsible for security of the areas in which the work is being performed prior to completion. 4.Vendor shall provide floor protection while working in all VA facilities. All material handling equipment shall have rubber wheels. 9. DELIVERY/STORAGE REQUIREMENTS 1.Deliver materials to job. 2.Package to prevent damage or deterioration during shipment, handling, storage and installation. Maintain protective covering in place and in good repair until removal is necessary. 3.Deliver specified items only when the site is ready for installation work to proceed. 4.Store products in dry condition inside enclosed facilities. 5.The Vendor shall accept a notification of delay as requested by the COR Interior Designer. 6.The Vendor shall coordinate this ordering arrangement at order placement with the COR Interior Designer. 7.All prices quoted at the placement and acceptance of order shall be firm fixed price. 8.All delayed delivery arrangements, within 60 days, shall be at no additional cost to the Government.
