Description

SF 542 Over $30,000 Vendor Set-Up Form W-9 New Vendor Registration Profile Subcontract and Statement of Work THIS IS A SUB-CONTRACTING OPPORTUNITY. The Cleveland Job Corps is currently seeking to subcontract a Mental Health Consultant to perform services for the students 20 hours per week. The Cleveland Job Corps Center is a training facility working with young adults aged 16-24. Our student body is continuously changing but maintains the level of 346 students enrolled. To be considered for award, a quote must comply in all material respects with the scope of work. Award will be made to the responsible contractors whose quote, conforming to the scope of work, is most advantageous to licenses, Certificate of Liability Insurance, W-9 & DUNNS #, Vendor In-put sheet, and Serrato 542 (If $30,000.00 or over) in your package. Please see the attached RFQ Scope of Work and Subcontract.
