Description
This is a combined synopsis / solicitation for commodities to purchase "Suicide Deterrent and Alarm System", (Brand Name or Equal / Lowest Price Technically Acceptable (LPTA)) for the Orlando VAMC at 13800 Veterans Way, Orlando, FL 32827. It is prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The combined synopsis / solicitation number is VA248-16-Q-0083. It's issued as a Request for quotation (RFQ). (iii) This combined synopsis / solicitation RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) 2005-83-3/11-01-2015. (iv) The combined synopsis / solicitation is Set Aside for Service-Disabled Veteran-Owned Small Business (SDVOSB). NAICS is 561621. Small business size standard is $15M. MARK FOR: Chandra Lillemoen 321-397-6881 Chandra.Lillemoen@va.gov ALL ITEMS TO BE DELIVERED TO THE ABOVE ADDRESS STATEMENT OF WORK The objective of this contract is to provide the Lake Nona Veterans Administration Medical Center located at 13800 Veterans Way with a contractual obligation that encompasses a "Suicide Deterrent and Alarm System" that detects the presence of a foreign object when placed over the top of a door before pressure can be applied and alerts caregivers of potential acts of self-harm. The deterrent device shall utilize photo electric presence detection technology. "A Site Visit is required in order to quote this job. Once solicitation is issued on November 23, 2015, contact Alberto Velazquez at Alberto.Velazquez-Santiago@va.gov to set schedule your Site Visit. The Site Visit available dates are December 7, 2015 to December 10, 2015. This contract shall not require access to a VA system or VA sensitive information (e.g., system administrator privileged access to a VA system, or contractor systems or processes that utilize VA sensitive information). The Contractor shall provide all labor, material, supplies, equipment and supervision necessary for execution of this contract. Contractor shall be responsible for the following: oFurnish alarm system on forty-four (44) doors. oOne complete electrified continuous hinge per door (44 each). oTwo sets of photo electric beams per door (88 sets). oOne key deactivation switch per door (44 each). oOne emergency notification strobe light per door (44 each). oOne 12' x 12' junction box w/ programmed input/output modules per door (44 each). oOne master control panel (1 each). oOne auxiliary power supply (1 each). oOne hallway siren (1 each). o2000' of Plenum rated cable. oEmergency power tie-in. oInstall EMT and wire from the interstitial for door detection device junction box, and control panel, master control panel, power supply and hallway siren. oAfter completion of work the contractor shall return all equipment and systems to operating condition. The Contractor is responsible for site clean-up and to remove and dispose of all trash and debris inclusive those that are combustible, flammable or explosive in nature. Contractor shall use clean drop cloths and other suitable coverings to ensure full protection to the areas in which the work is being performed. HOURS OF WORK: The hours of work shall take place during the normal business hours of 7:00 AM and 4:00 PM, Monday through Friday, Federal Holidays excluded. Request for weekend work must be submitted in writing. "Inspections" will require a prior five day notification submitted to the Contract Officers Representative (COR) or government representative. Contractor will be working in an unoccupied building and will be required to conform to the Pre-Construction Risk Assessment (PCRA) and requirements in the Construction Safety Standards. Contractor shall be responsible for site security. The facility shall remain occupied or locked at all times. In no case, shall the facility be left unlocked and unoccupied. Contractor shall coordinate with COR for access and deactivation of alarms. WARRANTY Contractor shall warrant that the equipment shall be free of defects in materials and workmanship arising from normal usage for a period of one year. (vii) The equipment is to be delivered on or before January 15, 2016 at the expenses of the offeror to Orlando VAMC, 13800 Veterans Way, Orlando, FL 32827. (viii) Provision 52.212-1, Instructions to Offerors - Commercial (JUL 2013), applies to this acquisition. "The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
