Wage Determination DM Local Information Performance Work Statement Linen Laundry Service Past Performance Report Form Past Performance Information Form 52.212-1 Pricing Worksheet Linen Laundry Service Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format prescribed in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation FA4877-16-Q-0090 is issued as a request for quotation (RFQ). This solicitation document incorporates provisions and clauses of those in effect through Federal Acquisition Circular 2005-84, Defense Circular Number 20150930, and Air Force Acquisition Circular 2015-1001. The Associated North American Industrial Classification Standard (NAICS) code for this request is 812331 with a size standard of $32.5M. The results of this requirement will be a firm fixed priced (FFP) contract plus four (4) Options with a Small Business Set-Aside. The resulting contract will consist of a base period and four (4) option periods; with a six (6) month extension period, if required. The right to make multiple or no award is reserved in the event it is advantageous to the Government to do so. GENERAL The completion and submission of the below items will constitute a quote and will indicate the vendor's unconditional assent to the terms and conditions of this RFQ and any attachments and/or exhibits hereto. •1. In addition to requirement of 52.212-1(b)(4) the offeror shall submit the following additional technical information: Offer must meet minimum requirements as stated in the Performance Work Statement (PWS). Provide a capability statement to include but not limited to (Industry certifications and Inspections conducted IAW PWS Appendix 3 Industry Regulations, Codes & Standards) of the offered services which will meet or exceed the minimum specifications as prescribed within the PWS. •2. In accordance with the requirement of 52.212-1-(b)(6) price and any discount terms: The total evaluated price will be inclusive of all CLINs for the 12-month base period, the 4-1 year option periods and the six month extension. The six month extension shall be priced by the offeror, IAW FAR Clause 52.217-8 using the last option year prices (Option Year Four). I f the extension is exercised, the price for the current option period will be pro- rated IAW FAR Clause 52.217-8 at the rates specified for that option period. •(a) Insert proposed unit prices in the aforementioned CLIN structure and schedule. The proposal(s) must be submitted for a base year and four (4) option years and the six-month extension. All Position Description Pricing shall be completed on the Pricing Worksheet in its entirety for consideration for award. II. QUOTE CONTENT Factor I - Past Performance Factor II - Price Quote Factor I - Past Performance The vendor shall demonstrate relevant past performance or affirmatively state that it possesses no relevant past performance. Relevant past performance is performance under contracts or efforts within the past five years prior to the solicitation closing date that is the same as or similar to, the scope and magnitude of the work described by this solicitation. To demonstrate its past performance, the vendor shall identify up to three (3) of its most relevant contracts or efforts within the past five (5) years and provide any other information the vendor considers relevant to the requirements of the solicitation. Vendors should provide a detailed explanation demonstrating the relevance of the contracts or efforts to the requirements of the solicitation. If subcontractor past performance is provided as part of the three (3) of its most relevant contracts or efforts, the subcontractor past performance will be given weight relative to the scope and magnitude of the aspects of the work under the solicitation that the subcontractor is proposed to perform. Therefore, the vendor's quote shall detail clearly the aspects of the work in the solicitation that the subcontractor is proposed to perform. The vendor should complete a "Past Performance Information Form" for each reference submitted. The form is an attachment to the solicitation. The forms will count toward the page limit described below. For additional information regarding a particular reference beyond that which will fit on the form, the vendor may continue onto another sheet of paper. Such continuation sheet(s) for submitted references will count toward the page limit. In addition to the information requested above, vendor shall contact their past performance references and request that each reference complete the attached "Past Performance Report Form" and e-mail the completed survey form directly to SrA Angel Hill at angel.hill@us.af.mil by the DUE DATE OF THIS SOLICITATION. The Government reserves the right to consider past performance report forms received after the due date of the solicitation and to contact references for verification or additional information. The past performance submission is limited to 12 pages. Factor II - Price Quote This factor shall include completed solicitation documents and additional supporting documentation described below. All Position Description Pricing shall be completed on the Pricing Worksheet in its entirety for consideration by the offeror. Separate pricing information shall be submitted for each year of the services as well as a total summary for base year and four (4) option years and the six-month extension clause. Unless completed in ORCA online, 52.212-3 "Offeror Representations and Certifications" shall be completed by the offeror and submitted with proposal. All price and price supporting information shall be contained in the price proposal. No price or pricing information shall be included in any other volume including cover letters. Offerors are responsible for submitting sufficient information to enable the Government to fully evaluate their price proposal. (End of provision) 52.212-2 - Evaluation -- Commercial Items (Oct 2014) Evaluation -- Commercial Items (Oct 2014) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: • i. Past Performance • ii. Price Addendum to FAR 52.212-2, Evaluation - Commercial Items (Oct 2014) The Government intends to award a Firm Fixed Priced (FFP) contract to the responsible vendor whose quote represents the best value after evaluation in accordance with the factors in the solicitation. The vendor's quote shall be in the form prescribed by, and shall contain a response to each of the areas identified in solicitation provision FAR 52.212-1 entitled "Instructions to Offerors-Commercial Items" and it's Addendum. The Government intends to award the contract without discussions. The establishment of a competitive range is not anticipated. Accordingly, each vendor should submit its most favorable terms from a price and technical standpoint. However, the Government reserves the right to establish a competitive range and conduct discussions if later determined by the Procuring Contract Officer (PCO) to be necessary. The evaluation of quotes will consider Factor I, Past Performance, to be more important than Factor II, Price. Even though Past Performance is more important than Price, initially, quotes shall be ranked from lowest to highest according to price, inclusive of all option pricing. A vendor's proposed prices will be determined by adding all extended amounts for the CLINs. If the lowest priced evaluated quote is evaluated to have a Substantial Confidence performance assessment, that quote represents the best value for the Government and the evaluation process stop at this point. Award shall be made to that vendor without further consideration of any other quotes. However, if the lowest priced vendor is not evaluated to have a Substantial Confidence performance confidence assessment, the next lowest priced vendor will be evaluated and the process will continue (in order of price from lowest to highest) until a vendor is evaluated to have a Substantial Confidence performance confidence assessment. At that time, the Government shall make an integrated assessment best value award decision between the quote rated with a Substantial Confidence and all lower priced vendors. Factor I - Past Performance For the Past Performance factor, the ratings identified in Tables 1 and 2 below, entitled "Past Performance Relevancy Ratings Table" and " Past Performance Confidence Assessment Ratings Table," respectively will be used for the assignment of ratings for relevancy and confidence assessment. Relevancy includes similarity in scope and magnitude. The vendor's past performance information will be evaluated to determine the quality and usefulness as it applies to performance confidence assessment. Past Performance will be assessed as follows: Evaluation will focus only on work experience already performed. Work yet-to-be performed, and work prior to the last 5 years, will not be considered. In addition, performance data will only be assessed for those references demonstrating at least 1 year of completed performance prior to the closing date of the solicitation. Subcontractor past performance will be given weight relative to the percentage of the work under the solicitation that the subcontractor is proposed to perform. Past Performance Relevancy Ratings - Regarding relevancy, each past performance reference under each vendor's Past Performance submission will be evaluated to determine its individual scope and magnitude relative to the instant requirement. The following definitions will apply to this evaluation: •· Scope: Experience in the areas defined in the Performance Work Statement (PWS). •· Magnitude: The measure of the similarity of the dollar value of actually performed work that exists between the PWS and the vendor's contracts. Vendors lacking relevant past performance history will not be evaluated favorably or unfavorably on past performance. However, the quote of an vendor with no relevant past performance history, while not rated favorably or unfavorably for past performance, may not represent the most advantageous quote to the Government. In this instance, the vendor will receive a rating of "Not Relevant" in the relevancy rating factor. Past Performance Confidence Assessment Ratings - The overall assigned rating for Past Performance will be the Past Performance Confidence Assessment rating. The assignment of this rating will be based on the quality of the relevant past performance and will consider the currency and relevance of the information, source of the information, context of the data, and general trends in contractor's performance. The quality of performance under a past performance reference that has no relevance to the instant requirement will not be considered in the overall assessment of Past Performance Confidence. In the case of a vendor without a record of relevant past performance or for whom information on past performance is not available, the vendor may not be evaluated favorably or unfavorably on past performance rather the vendor will receive an "Unknown Confidence" rating. In order to verify past performance information and determine the quality of the past performance submission, the Government may contact some or all of the references provided, as appropriate, and may collect information through questionnaires (i.e. the Past Performance Report Form), telephone interviews and existing data sources to include but not limited to Contractor Performance Assessment Reporting (CPARS). The Government reserves the right to obtain information for use in the evaluation of past performance from any and all sources including sources outside of the Government. This past performance information will be used for the evaluation of past performance. This evaluation and rating is separate and distinct from the Contracting Officer's responsibility determination. The assessment of the vendor's past performance will be used as a means of evaluating the relative capability of the vendor and other competitors to successfully meet the requirements of the RFQ. In determining the rating for the past performance evaluation sub-factor, the Government will give greater consideration to the contracts which the Government feels are most relevant to the RFQ. Factor II - Price •· Price quotes will be evaluated in accordance with FAR 13.106-3(a), Award and Documentation. •· For the purpose of preparing a price quote, the vendor shall assume that the period of performance will be 12-months with four (4) 12-month option periods. The Government will evaluate the price quote for the base period and each option period. •· The Government may evaluate any and all information submitted by the vendor to support the reasonableness of prices proposed. The method of evaluation used by the Contracting Officer is solely within the discretion of the Contracting Officer. •· Options, to include FAR 52.217-8, will be evaluated pursuant to solicitation provision FAR 52.217-5, Evaluation of Options. The Government will evaluate quotes for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that a quote is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). •· Although price is the not the most important evaluation factor, it has the potential to become more significant during the evaluation process. The degree of importance of price will increase with the degree of equality of the quotes in relation to the other factors on which selection is to be based. The importance of price will also increase when a quote's price is so significantly high as to diminish the value to the Government that might be gained under the other aspects of the quote. If, at any stage of the evaluation, all vendors are determined to have submitted equal, or virtually equal, non-price quotes, price could become the factor in determining which vendor will receive the award. Written Notice A written notice of award or acceptance of a quote e-mailed or otherwise furnished to the successful vendor within the time for acceptance specified in the quote shall result in a binding contract without further action by either party. Before the quote's specified expiration time, the Government may accept a quote (or part of a quote), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Limited Confidence Based on the vendor's recent/relevant performance record, the Government has a low expectation that the vendor will successfully perform the required effort. No Confidence Based on the vendor's recent/relevant performance record, the Government has no expectation that the vendor will be able to successfully perform the required effort. Unknown Confidence (Neutral) No recent/relevant performance record is available or the vendor's performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned. The Government intends to evaluate quotations and award a contract using the acquisition procedures of FAR 13.5. The Government will select the vendor whose quote represents the best value to the Government, considering price and other factors when compared to other vendors. The Government also reserves the right to not award a contract or order if the award is not in the best interest of the Government. The FSRS website can be found at www.FSRS.gov. The Contracting Officer is responsible to "review" the information provided in FSRS. The prime contractor shall notify the Contracting Officer after submission of the report via email at Jonathan.Hollis@us.af.mil. Any questions pertaining to this acquisition shall be asked no later than Wednesday, 9 December 2015, 10:00 AM, Local Arizona Time. Questions shall be sent to SrA Angel Hill via email at angel.hill@us.af.mil and SrA Christopher Vilmont via email at christopher.vilmont@us.af.mil. It is the responsibility of the offeror to view the website for any changes or amendments to this RFP. Quotes will be due to the 355th Contracting Squadron, 3180 S. Craycroft Rd.
