- REQUEST FOR INFORMATION Cardiovascular Prefusionist Services, Richard L. Roudebush VAMC, Indianapolis, IN This is a Request for Information. This advertisement is not a solicitation for offers, nor is it a Request for Quotations (RFQ). An RFQ may be issued at a later date. The purpose of this RFI is to conduct market research to determine if responsible sources exist and if they are capable of providing Cardiovascular Prefusionist services at the Richard L. Roudebush VAMC, in Indianapolis, IN. The proposed North American Industry Classification is 621399, Offices of all Other Miscellaneous Health Practitioners ($7.5 Million). The requirement is for.4 FTE Perfusionist to perform services two days a week (8:00 am to 4:30 pm) for a total of 832 hours a year. Eighty (80) hours of call back coverage will be expected to cover emergent cases. The requirement also includes a maximum of 7848 on-call hours during non-duty hours. The period of performance is projected for 1 April 2016 thru 31 March 2021 (a base year with four, one year option periods). The Perfusionist(s) shall be available to the Indianapolis VAMC for perfusion-related procedures such as open-heart surgical procedures, procedures requiring auto-transfusion, and intra-aortic balloon device troubleshooting. Perfusionist(s) shall also be available on an as-needed and/or emergency basis. The assigned Perfusionist(s) shall be well versed in all current technologies related to perfusion services such as percutaneous cannulation for minimally invasive and robotic surgery, and implanting and managing ventricular assist devices (VAD) and angio vac procedures, as well as vascular, urology and any other specialty that may require perfusion services. He/she shall work closely with the surgeons and anesthesiologists with respect to management of complex Congestive Heart Failure (CHF) cases and redo cardiac surgeries, as well as aortic surgery. Basic duties include, but not limited to: (1) Set up and operate cardiopulmonary bypass equipment, conduct cardiopulmonary bypass, monitor and document all patient perfusion parameters, and make necessary adjustments for routine and emergent procedures requiring cardiopulmonary support. (2) Operate all associated perfusion equipment including cardioplegia, cell saver, intra-aortic balloon pump (IABP), ventricular assist device (VAD) and femoral-femoral cardiopulmonary support (emergency bypass) as necessary. (3) Administer medications and blood components via bypass as necessary after consultation with the attending cardiac surgeon or anesthesiologists. (4) Assist in trouble-shooting IABP problems. (5) Provide comprehensive documentation of perfusion related services in the patient's medical record. (6) Clean all perfusion equipment at the end of the procedure, conforming to all relevant hospital policies. (7) Maintain inventory levels and request supply replacements through established VA processes. (8) Promote patient safety before, during, and after the operation. (9) Maintain aseptic technique. (10) Assist in the evaluation of new products and procedures. (11) Shall have current Basic Life Support Certification (BLS). This RFI is for the following: 1. To gage the market and determine if sufficient small business entities are capable of providing the required services. 2. To request innovative and unique solutions for performing 24/7-365 on call services. 3. To determine if there are cost-effective methods for performing the requested hours based on the CLIN structure below. 4. To request alternate examples of the proposed cost/price schedule.
