Description
The Regional Health Contracting Office - Atlantic, is currently seeking sources for a requirement to provide McDonald Army Health Center (MAHC), Building 576, Fort Eustis, VA with a Ossa Tron high energy electro-hydraulic extracorporeal shockwave device. This is a Sources SOught Notice only. Solicitations are not available at this time. The Government anticipates the procurement will be solicited as Unrestricted. Market research is being conducted to determin interest and availability of potential sources for this requirement. Below is a draft Performance Work Statement. Performance Work Statement Leased OssaTron, McDonald Health Care Clinic, Fort Eustis, VA 1. General: This will be a non-personnel service contract lease for a OssaTron high energy electro-hydraulic extracorporeal shockwave device for usage at McDonald Army Health Center on a bi-monthly basis for dates to be determined by the Operatory Chief or Operatory Nurse OIC based on scheduled Ortho/Podiatry surgeries. 1.1 Description of Services and Introduction: As part of the lease agreement, contractor shall provide all personnel, supplies, equipment, supervision, transportation, and other items and non-personal services necessary to support equipment usage at McDonald Army Health Center (MCAHC) campus at Fort Eustis and as defined. The contractor shall perform to the standards within the contract. 1.2 Background: MCAHC has the overall responsibility to ensure that all of its patients receive the best health care treatment. By incorporating the use of the OssaTron within the operatory suites, MCAHC can alleviate the waiting list of patients awaiting treatment for multiple tendinopathies (includes: chronic plantar fasciitis and chronic lateral epicondylitis). 1.3 Scope: 1.3.1 The contractor shall provide all necessary labor and transportation to properly deliver the OssaTron and accessories to MCAHC, Building 576, Operatory Suites, Fort Eustis, VA. 1.3.2 Equipment must be mobile, self-contained and clean upon delivery. 1.3.3 Contractor's employees will directly coordinate drop-off and pick-up dates of equipment with the Operatory Chief, Operatory Nurse or Operatory NCOIC between the hours of 7:30 am - 3:00 pm, Monday through Friday. Holidays excluded. 1.3.4 The contractor will be responsible for all direct maintenance cost of the equipment during the period of performance. 1.4. Period of Performance (POP): 15 December 2015 through 14 December 2016.
