Attachment 1: Pictures from Marine Inspection Survey Solicitation with the Submission Instructions and Submission Requirements SUBMISSION OF QUOTES: The U.S. Army Corps of Engineers - Baltimore District has a requirement for the repair and maintenance of the BD-5 Vessel. The Government is using FAR Subparts 12 and 13 and will award a firm-fixed-priced contract resulting from this solicitation to the responsible contractor whose quote conforms to the solicitation. The Government anticipates award of a Firm-Fixed-Price (FFP) Lowest Price (Cost) Technically Acceptable contractor, i.e. to the contractor with the lowest evaluated realistic price of quotes meeting the acceptability standards for non-cost factors. To receive consideration for award, a rating of no less than "Acceptable" must be achieved for the Technical factor. Award will be made with appropriate consideration given to the three evaluation factors: Factor 1-Technical, Factor 2 - Past Performance, and Factor 3-Cost/Price. The Government intends to evaluate quotes and make award without questions to contractors. Therefore, the initial quote should contain the contractor's best terms from a Technical and Cost standpoint. Unclear quotes may not result in questions for clarifications. Offers with an omission or unclear quote may be judged to mean the contractor does not fully understand the requirements or understand what it takes to meet the requirements, regardless of the proposed cost/price and will be rated unacceptable. In that case, the contractor may be found unacceptable, and ineligible for award. The Government reserves the right to ask questions of contractors if the Contracting Officer determines them to be necessary. Offerors must be registered on the System for Award Management (SAM) to be eligible for award. The Government reserves the right to award without discussions. In order to be eligible for award, a contractor must be determined to be responsible in accordance with FAR 9.104. Quotes shall be submitted via e-mail to tamara.c.udasco@usace.army.mil ATTN: Tamara Udasco, no later than the time and date specified on Standard Form 1449, Block 8. Unclear quotes may not result in questions for clarifications. Quotes received after the required time and date deadline will not be evaluated. QUESTIONS Inquiries to end users or any personnel regarding this project are not permitted. Please direct any inquiries concerning this solicitation must be submitted by email no later than Tuesday, December 8, 2015 at 12:00PM (EST). Email: tamara.c.udasco@usace.army.mil. No questions will be taken after that time. No assumptions, conditions, caveats or exceptions submitted with quotes in response to this solicitation will be accepted or incorporated into the awarded contract. Any and all assumptions must be addressed and/or resolved through the use of the question and answer period specified above. The offer shall be clearly indexed and logically assembled. All pages shall be appropriately numbered and identified by the complete company name, date and solicitation number in the header and/or footer. Factor 1 - Technical shall not exceed 25 pages; Factor 2 - Past Performance: See Questionnaire for more instruction and not to exceed 10 pages; Factor 3 - Price - no page limit. All files shall be submitted as either a Microsoft Excel (.XLS) file, Microsoft Word, or Acrobat (PDF) file as indicated in the table. SUBMISSION REQUIREMENTS: Quotes shall contain the following: Cover letter on quoter's letterhead, summarizing the pricing total and outlining quote terms and conditions, signed by the representative managing the project. Include Cage Code, DUNS # and Tax ID. Price Schedule completed with all pricing. a) Introduction: The contractor's quote shall be submitted via electronically to tamara.c.udasco@usace.army.mil. NOTE: Files shall not contain classified data or hyperlinks. b) Each contractor is required to submit a quote consisting of three (3) volumes.  Volume 1 - Technical (no pricing information shall be included) - Technical submissions shall include the following and comply with the US Army Corps of Engineers Safety & Health Requirements Manual EM 385-1-1 (dated 30 November 2014): o Work Plan - Provide a written plan of means, methods, manpower and equipment for conducting the work according to the Performance Work Statement. Quote shall provide enough information to determine whether the contractor's methods and approach to meeting the requirement provides the Government with a high level of confidence of successful completion. The evaluation will also consider the realism of the allocated hours, labor categories, materials, and item prices being quoted. o Commercial Warranty.  Volume II - Past Performance (No pricing information shall be included) PAST PERFORMANCE SUBMISSION REQUIREMENTS: Past performance information is required for this solicitation. Past performance information refers to the Contractor's recent and relevant projects, as well as to the quality of the experience from the Customer's perspective. In this solicitation, "recent" is considered to be within the past three (3) years, and "relevant" is considered to be "of similar size, scope and complexity" to the current requirement. The Contractor shall: a) Complete the table below for two (2) recent and relevant projects. In the last row of the table, provide contact information (name, organization, phone number and email address)for a Customer who is able to provide a reference for the specific project identified. b) Provide a narrative, for each project listed in the table, which describes the project and outlines the relevancy of the project to the current Performance Work Statement (PWS). All similarities to the current PWS shall be highlighted in yellow in the narrative. c) Send the Past Performance Questionnaire (PPQ), provided as part of this solicitation, to each of the references identified and request that the reference complete the PPQ by the deadline stated in Box 8 of the solicitation. The completed PPQ's must be returned by Customer directly to Tamara Udasco, Contract Specialist, e-mail at tamara.c.udasco@usace.army.mil. The Government will not release completed questionnaires to the contractor at any time, in order for the Government to solicit candid, unbiased responses and comments. d) Contractors without recent or relevant past performance information should clearly state this in the quote and refer to the "Evaluation" section for additional information. ( Please see the table listed in the solictiation). Volume III - Cost/Pricing. No pricing information shall be in the Technical Volume submission. Contractors shall include a Price Schedule complete with all pricing, inclusive of installation, material, project management and other cost. Note: Material and installation should be for the total project as this will be a fixed-price contract. Each contractor should also include a cover letter on company letterhead, summarizing the pricing total and outlining the contractors terms and conditions, signed by the representative managing the account. Include the contractor's CAGE Code, TAX ID #,DUNS #, and SIN #. The total price shall be equal to the sum of the total of all of the cost elements (Specified Items, Labor, and Materials). The Government will verify the contractor's calculation of the total offered price. The prime contractor and each subcontractor shall provide their labor rates for each of the labor categories contained. Each labor rate shall be multiplied by its corresponding level of effort (hours) for the contract period of performance. The contractor shall ensure that the rates proposed for all of the labor categories subject to the Service Contract Act (SCA) comply with the minimums specified by the applicable Department of Labor Wage Determination. Each volume shall be submitted via separate files. Any pages that are changed (as a result of quote revisions) shall be dated. Each volume shall be clearly named with its Title, Submission Date, Solicitation Number and copy number. The following volumes of material shall be submitted Volume I- Technical, limit 25 pages via email. (No pricing information shall be listed). Volume II - Past Performance, limit 10 pages via email. Volume III - Price, No page limitation via email. Specific Requirements: A quote will consist of all information and material submitted via email for evaluation in response to this RFQ. The submission of false or misleading information will be grounds for disqualification of the quote. Information requested that is missing may be ground of disqualification
