Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number VA249-16-Q-0128 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83. The requirement is a Small Business (SB) set-aside. The associated NAICS code is 621511 and small business size standard $32.5 Million. The government anticipates awarding a Firm Fixed Price award. Award will be made to lowest price quote which conforms to the requirements within this solicitation. The Memphis VAMC, Pathology & Laboratory Service, located at 1030 Jefferson Avenue, Memphis, TN 38104 needs a contract for base + (4) one-year options to renew for T-Spot TB Testing. The contract period of performance shall start the date of award through 2020. CLIN 0001: Base Year - T-Spot TB Testing CLIN 1001: Option 1 - T-Spot TB Testing CLIN 2001: Option 2 - T-Spot TB Testing CLIN 3001: Option 3 - T-Spot TB Testing CLIN 4001: Option 4 - T-Spot TB Testing Also, review the attached statement of work (SOW), Price/Cost Schedule, FAR clauses & provisions, IT Checklist and Wage Determination 05-2493 for complete details. The Contractor shall submit a quote on company letterhead and include the following information: name, address, and telephone number and DUNS#. The quote shall have the following information included: quantity, unit frequency, unit price, total cost, applicable shipping charges, offered discounts or proposed delivery listed in the item description as specified above. 2. The Contractor shall complete the annual representations and certifications electronically via the SAM Web site accessed through https://www.SAM.gov. 3. The Contractor shall provide sufficient evidence that they possess adequate resources, capability, experience, responsibility and integrity to meet the technical capabilities to comply with the requirements of the solicitation. 4. The Contractor shall provide past performance evidence. This assessment is based on the Contractor's record of relevant and recent past performance information that pertains to services outlined in the solicitation requirements. 5. The Contractor shall provide a copy of the appropriate licenses/certifications pertaining to their current State Laboratory License program. 6. All contractor personnel shall have current license/certifications pertaining to their current State License program. The Contractor shall be able to provide a copy of personnel licenses/certifications after contract award to the COR or CO. 7. All questions must be submitted by 8am CST, Thursday, December 3, 2015.
