VA248-15-Q-2180 AMENDMENT 1 The purpose of this amendment is to answer the following questions. Question: The RFQ mentions Ultrasound and MRI equipment however there is no mention of how many assets there are. How many of each should be included in our response? Answer: The SOW is inclusive of the number of Ultrasound and MRI assets, found in section K and section F of the SOW. Question: On page 28, item 1.4 requests us to submit the labor mix, how should we fill out this table? 1.4Offer must submit their labor mix - (See Table Under Schedule) supply the labor classes and fully burdened rates both regular and overtime to conduct this service by year to include all options. This can be provided in excel format. This being a highly regulated field there may be new regulations and or standards that may need to be met that may require the Government to purchase on an emergent basis. The vendor would price emergent services with the use of this table. The Labor Mix is defined as the types of personnel that would be required to provide the Physicist On-Site Services. Question: Does the Government have a suggestion for a way we can know that the Past Performance Questionnaire forms have been submitted? Answer: The contractors may be carbon copied by individuals completing the past performance questionnaire, therefore knowing it has been submitted properly to the Government. Question: In regards to the written reports, historically they have been provided in word or excel format, is that acceptable? Answer: Yes Question: The Government asks for a hard copy binder at the end of the contract. Could the vendor supply a CD with all of the documents on it, is a hard copy required? Answer: No, a CD would suffice for this documentation requirement. Question: The ACR accreditation testing, the PET and SPEC are tested annually under the ACR standard however our scope asks for twice a year and quarterly testing respectively. Answer: For clarification, the vendor is required to complete ACR accreditation testing at least annually with semi-annual and quarterly results assessment of the testing performed by our nuclear medicine technologists, respectively.
