Description

The Department of Veterans Affairs Northern California Health Care System plans to issue a Request for Quote (RFQ) to obtain outpatient services in the Yreka, CA area. The contractor shall furnish all necessary facilities, equipment, materials and personnel to provide a Community Based Outpatient Clinic (CBOC) and Primary Care Services within 30 miles of Yreka, California to the Veterans Affairs Northern California Health Care System (VANCHCS) eligible veteran patients. The Contractor must have the necessary professional medical staff, diagnostic testing and treatment capability and referral arrangements needed to ensure continuity of health care for enrolled VA patients. Services include primary care, pharmacy services, laboratory services, radiology services, and emergency mental health services. NAICS code 621498 applies with a size standard of $20.5 million. This solicitation will be a small business set aside acquisition. The contract will be a fixed price type contract and is expected to include a one-year basic period, with a one-year option period. The solicitation number for this RFP is VA261-16-Q-0130. The Solicitation will be issued o/a December 8, 2015, with an anticipated proposal due date of January 6, 2016. The Offeror is responsible for downloading all documents including amendments at the website listed above, without further notification from the VA Contracting Officer. Contractors must be registered in the System for Award Management (SAM) at www.sam.gov prior to award of a contract.
