Description
This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA249-16-Q-0142 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. This requirement is Total Service-Disabled Veteran-Owned Small Business Set-Aside. The NAICS is 561720 and the small business size standard is $18.0 Million. In accordance with VAAR 852.219-11, any SDVOSB submitting a quote for this action must be verified for ownership and control and is so listed in the vendor information pages database (http://www.vetbiz.gov) - upon submission of quote. Only qualified offerors may submit bids. The Service Contract Act is applicable to this solicitation. Reference the following link for Wage and Determinations for WD 05-2495 (Rev.-19) http://www.wdol.gov/wdol/scafiles/std/05-2495.txt?v=16 Introduction: The government anticipates awarding a Firm Fixed Price award, lowest price technically acceptable (LPTA). The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation requirements. Description of Services: The Memphis VA Medical Center has a requirement for a contractor to provide all management, labor, supervision, supplies, tools, equipment, materials, and quality control to carry out refinishing services to the wood stage floor in Room CW149 at the VA Medical Center Memphis, 1030 Jefferson Avenue, Memphis, TN 38104. The contractor shall perform to the standards in the contract as well as all local, state, and federal regulations. See the Statement of Work (SOW) for complete details. Place of Performance: Memphis VA Medical Center, 1030 Jefferson Avenue, Memphis, TN 38104 Period of Performance: All work completed within 120 calendar days after Notice to Proceed Site Visit: A Site visit is available on Monday, December 14, 2015 at 2:00pm CST. Full text can be obtained at http://www.acquisition.gov/far/index.html LIMITATIONS ON SUBCONTRACTING-- MONITORING AND COMPLIANCE This solicitation includes VAAR 852.215-70, Service-Disabled Veteran-Owner and Veteran-Owned Small Business Evaluation Factors, and VAAR 852.215-71, Evaluation Factor Commitments as outlined in the solicitation. Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an "Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement" to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the contractor to protect proprietary information as required by FAR 9.505-4, obtaining access to proprietary information, paragraph (b). The contractor is required to cooperate fully and make available any records as may be required to enable the CO to assess the contractor's compliance with the limitations on subcontracting or percentage of work performance requirement. Submission of Quote: The offeror shall shall submit their quote on company letterhead and shall include unit price, total, unit qty and item description as specified above, as offered discounts, proposed delivery time, name, address, and telephone number of the offeror, firm's DUNS# and ORCA document in SAM at www.sam.gov, terms of any express warranty, unit price, overall total price, applicable shipping charges, completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and ORCA document. All prospective bidders must include appropriate references which must include all applicable company information. The offeror should provide past performance evidence. The past performance evaluation results is an assessment of the offeror's probability of meeting the solicitation requirements. This assessment is based on the offeror's record of relevant and recent past performance information that pertain to the products and/or services outlined in the solicitation requirements. Past performance information may be obtained through other sources known to the VA and the Federal Government. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past. Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability "unknown" shall be considered "acceptable." STATEMENT OF WORK 1.DESCRIPTION OF SERVICES: The Contractor shall provide all management, labor, supervision, supplies, tools, equipment, materials, and quality control to carry out refinishing services to the wood stage floor in Room CW149 at the VA Medical Center Memphis. All work performed shall be performed in accordance with all applicable laws, regulation, standards, instructions, and commercial practices. 2.REQUIREMENTS: a.The Contractor shall provide material and labor to refinish existing wood stage floor at the Memphis VA. b.The Contractor shall remove existing ¼ round trim around stage to protect from sanding machine. c.The Contractor shall sand existing wood floor and ssmoothen out steps. d.The Contractor shall apply 3 coats of water based polyurethane to the sanded areas. e.The Contractor shall reinstall existing ¼ round and repaint base trim and ¼ round. f.The Contractor shall perform the removal of metal stair railings for refinishing and replace it prior to the completion of work. g.The Contractor shall clean work area upon completion of project. h.The Contractor shall vacuum carpeted area around stage to esnure cleanliness. i.The Contractor shall ensure that only FULLY QUALIFIED TECHNICIANS/WORKERS/ETC. are employed in the performance of any and all work performed under this contract. j.The Contractor shall at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this Statement of Work when the Government facility is not closed for the above reasons. k.When hiring personnel, the Contractor shall keep in mind that the stability and continuity of the workforce are essential. l.The Contractor shall ensure that the highest standard of professional capability and workmanship is maintained throughout the life of this contract. 3. SQUARE FOOTAGE: a.The stage floor is approximately 1300 sq. ft. b.There is no floor plan available for the theater stage; overall the total sq. ft. is approximately 3442 sq. ft. 4. HOURS OF OPERATION: The work provided by contractor must meet the actions listed in the schedule of work. All work performed must be between the normal working hours of 8:00 am to 4:00 pm Monday through Friday. The facility will be closed on all federal holidays. Work cannot be performed during holidays or after hours without consent from the facility, contracting officer, and/or contracting officer's representative. The following is a list of U.S. Government holidays. If the holiday falls on a Saturday, the proceeding Friday is observed as the holiday; if the holiday falls on a Sunday, the following Monday is observed as the holiday. Engineering should be notified of all individuals working onsite. Employees must have a valid state or government identification. All contractor/vendor personnel shall display a VAMC Memphis issued identification badge while performing work on any VAMC property. b. Smoking is not prohibited on property except in designated areas. Possession of weapons is prohibited (in vehicle or on person). Enclosed containers, including tool kits, shall be subject to search. Violations of the VA regulations may result in citation answerable in the United States (Federal) District Court. c. Contractor employees (crew) when on VAMC Memphis grounds; shall not bring visitors into the facility; shall not bring into the facility, any form of weapons or contraband; shall not bring in any alcohol or drugs or be under the influence of alcohol/drugs; shall conduct themselves in a professional and polite manner at all times; shall not cause any disturbances in the facility, are subject to all the rules and regulations of the VAMC Memphis. Contractor personnel shall present a neat appearance and be easily recognized. Contractor shall furnish and require every on-site company employee to wear a company shirt and safety shoes. All company workers while on duty shall be identified by uniform shirt or blouse indicating the company logo. The print should be large enough to be easily read. In addition all company staff assigned to work at the VAMC Memphis. d. The Contractor shall be responsible for safeguarding all Government equipment, information and property provided for Contractor use. At the close of each work period, Government facilities, equipment, and materials shall be secured. 6. PLACE OF PERFORMANCE: Department of Veterans Affairs VA Medical Center Memphis, 1030 Jefferson Street, Memphis, TN 38104 7. PERIOD OF PERFORMANCE: All work must be completed within 120 calendar days after Notice to Proceed. This is due to theater scheduling and the time available to work in the theater. 8.SAFETY: a.The contractor shall ensure that appropriate personal protective equipment (PPE) is worn at all times. b.The contractor shall abide by all regulations and safety requirements of the VA such as OSHA, NFPA, EPA, etc. 9.PARKING: The Contractor's staff will not park in patient parking areas. Parking is limited on the grounds and is first come first serve. 10.SITE REGULATIONS: The contractor shall ensure that all work shall be performed in accordance with VAMC Memphis rules & regulations.
