The Government is not obligated, nor shall it pay for, any information received from potential sources as a result of this Sources Sought Notice. The Orlando VA Health System is seeking business concerns capable of providing HD Digital Television and Business Class Dedicated Internet Access (DIAS) Coax jacks and DIAS connectivity Service for delivery of local and cable entertainment channels for the Orlando VA Medical Center Facility addresses are listed below: Orlando VA Medical Center (Lake Nona Campus) 13800 Veterans Way Orlando, Fl 32827 Lake Baldwin Campus (Orlando OPC) 5201 Raymond Street Orlando, Fl 32803 William V. Chappell Jr. Outpatient Clinic (Daytona OPC) 551 National Healthcare Drive Daytona Beach, Fl 32114-1495 Viera Outpatient Clinic 2900 Veterans Way Viera, Fl 32940 Clermont 805 Oakley Seaver Drive Clermont, Fl 34711 Crossroads Annex 925 South Semoran Blvd Suite 114 Winter Park, Fl 32792 Kissimmee Community Based Outpatient Clinic 2285 North Central Avenue Kissimmee, Fl 34741 Leesburg Community based Outpatient Clinic 711 W. Main Street Leesburg Fl 34748 Orange City Community Based Outpatient Clinic 2583 South Volusia Ave (17-92) Suite 300 Orange City, Fl 32763 The new Orlando VA Medical Center is located on a 65-acre campus in southeast Orange County (Medical City). The medical center will have a large multi-specialty outpatient clinic, 96-Acute Care beds, 120-community living center (CLC) beds, a 60-bed domiciliary (DOM), Chapel and administrative and support services. In addition to the Lake Nona (Medical City) location, the medical center is also responsible for the Out Patient Clinics (OPC) and Community Based Out Patient Clinics (CBOC) listed above as well. Facilities Management Service has a requirement for cable based service providing both off-air (local) and nationally distributed channels. This programming will significantly improve the quality and quantity of choices available to our Veterans. As part of the technological innovations being implemented the Corporate Orlando VA Medical System will be installing a facilities management public internet platform and broadcast system providing internet services and broadcast television to patients. All locations above will need DIAS and Video outlets throughout these facilities itemized by facility below will receive the television- Cable service, high speed and symmetrical connection to the internet up to 1 Gbps. The contractor is required to have infrastructure in place at all facilities listed above and will be able to provide uninterrupted service to all locations. Contractor is required to have point to point/end to end MetroE DIAS at all facilities listed on page one as well as Infrastructure permission and installation for Lake Nona (Medical City). This requirement will not be a multiple award contract. Firms with interest and capability which already have the infrastructure and capabilities (To include: Large business, Federal Supply Holders, Small Business, Veteran-Owned Small Business, Service-Disabled Veteran-Owned Small Business, HUBZone Small Business, Small Disadvantaged Business, and Women-Owned Small Business Concerns) may respond by submitting a qualifications letter outlining their qualifications to include the following information: 1.Name 2.Capability Statement (to include the Counties/Locations of interest) 3.DUNS # 4.Type of business (i.e., SDVOSB, VOSB, WOSB, Large, etc ¦) Responses to this sources sought notice shall be sent via email, no later than 11:00 AM December 31, 2015.
