Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number VA249-16-Q-0166 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83. The requirement is a Small Business (SB) set-aside. The associated NAICS code is 238220 and small business size standard $15 Million. The government anticipates awarding a Firm Fixed Price award. Award will be made to lowest price quote which conforms to the requirements within this solicitation. The Memphis VAMC, Engineering Department (138), located at 1030 Jefferson Avenue, Memphis, TN 38104 needs a contract to replace and install (73) Hot Water System Balancing Valves. Also, review the attached statement of work (SOW), Price/Cost Schedule, FAR clauses & provisions, and Wage Determination 05-2495 for complete details. The contract period of performance: Date of Award thru 1/31/2016. The Contractor shall submit a quote on company letterhead and include the following information: name, address, and telephone number and DUNS#. The quote shall have the following information included: quantity, unit frequency, unit price, total cost, applicable shipping charges, offered discounts or proposed delivery listed in the item description as specified above. 2. The Contractor shall complete the annual representations and certifications electronically via the SAM Web site accessed through https://www.SAM.gov. 3. The Contractor shall provide sufficient evidence that they possess adequate resources, capability, experience, responsibility and integrity to meet the technical capabilities to comply with the requirements of the solicitation. 4. The Contractor shall provide past performance evidence. This assessment is based on the Contractor's record of relevant and recent past performance information that pertains to services outlined in the solicitation requirements. 5. All questions must be submitted by 8am CST, Wednesday, December 30, 2015.
