VENDOR QUESTIONNAIRE AND W-9 SCOPE OF WORK "THIS IS A PROCUREMENT OPPORTUNITY WITH ADAMS AND ASSOCIATES, INC., AT THE ST. LOUIS JOB CORPS CENTER AND NOT WITH THE FEDERAL GOVERNMENT" 　 Scope of Work for: St. Louis Job Corps Center Operated by Adams and Associates, Inc. For the GREASE TRAP/SEWER SERVICES Prepared by: Marian Dillard 　 Scope of Work ID: STLJC2016-004 　 　 　The St. Louis Job Corps is requesting quotes for cleaning of the grease trap/sewer services. Contractor shall pump and pressure wash the grease interceptors. The lids should be siliconed when finished Contractor is to return written bids along with all licenses for doing business in the State of Missouri, certificate of Liability insurance and list of references if no past experiene with Center.
