Description
Statement of Work, Revision 1, All occurrences of 'Red Brass' have been replaced by 'Brass 'and material composition requirements have been removed. Please read through attached SOW for additional details. SF30, Amendment 0002 (i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii)Solicitation number FA3047-16-R-0004 is issued as a Request for Proposal (RFP). (iii) This RFP incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-85 and DFARS Change Notice 20151130. (iv) This acquisition is a 100% Small Business Set-Aside under NAICS Code 332999 with a size standard of 500 employees. (v) Please see attached SF 1449 for Contract Line Items, quantities and units of measure. (vi) This is a requirements contract for filling all of the agency's requirements for finished metal (United States Air Force) USAF name tags, (In Accordance With) IAW (The Institute of Heraldry) TIOH specifications for approximately 37,790 trainees or prior service personnel per year at (Joint Base San Antonio) JBSA Lackland AFB, TX. The metal name tags are designed for wear on the Air Force Service Dress Uniform. Name tags shall state the last name of the trainee or prior service personnel. Each trainee or prior service personnel will receive one (1) each USAF Service Coat Metal Name Tag. The required delivery is two weeks (14 days) from the time an order is placed to the contractor. The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items necessary to make metal USAF name tags. Name tags shall fully comply with AFI 36-2903. All orders shall be delivered to 502LRS/LGRM, 1550 Wurtsmith Suite 4, Bldg. 5725, JBSA Lackland AFB, TX 78236-5250. This RFP is NOT to be construed as a commitment by the United States Air Force. The Government is NOT liable for any costs and/or activity associated with providing a response to this announcement. Any costs incurred as a result of a response to this announcement shall be borne by the firm and will not be charged to the government for reimbursement. The Government reserves the right to award some or all of the line items represented herein. The Government reserves the right to cancel this solicitation either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse for costs associated with providing a quote. (vii) All orders shall be delivered to 502LRS/LGRM, 1550 Wurtsmith Suite 4, Bldg. 5725, JBSA Lackland AFB, TX 78236-5250. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, apply to this acquisition. Please see Addendum to FAR 52.212-1, Instruction to Offerors for additional instructions. (ix) The provision at 52.212-2, Evaluation -- Commercial Items applies to this acquisition. Please see Addendum to FAR 52.212-2, Evaluation for additional evaluation criteria. (x) All offerors shall include a completed copy of the clause at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. (xi) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. (xii) The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition. (xiii) All customary commercial warranties apply to items provided under this solicitation. (xiv) Defense Priorities and Allocations System: Not Applicable (xv) Proposals and samples resulting from this solicitation are due no later than 4:30 P.M. Central Time on 22 January 2016.
