- Attachment 4 - Past Performance Questionnaire Attachment 3 - Evaluation Criteria Attachment 2 - Proposal Instructions Attachment 1 - Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 and 13.5, as supplemented with additional information included in this notice. This attached Request for Quotation (RFQ) document incorporates those provisions and clauses which are in effect through Federal Acquisition Circular 2005-85. The RFQ number associated with this combined Synopsis/Solicitation notice is DJA-16-ALAB-PR-0045. The Government anticipates a firm-fixed price award as a result of this notice. The NAICS code for this requirement is 611699 and a size standard of $11,000,000. This requirement is being solicited as a total small business set-aside. All interested parties must be registered in the System for Award Management (SAM) system before award of any BPA in accordance with FAR 52.204-7. FAR 52.212-4 and 52.212-5 apply to this notice. Also, any applicable local ATF clauses will be incorporated into the awarded BPA. Description of Service: The Bureau of Alcohol, Tobacco, Firearms and Explosives, Office of Science and Technology, National Firearms Examiners Academy has a requirement for qualified instructors to ensure prompt and effective forensic firearm identification training. The period of performance for each awarded BPA is date of award for five years (60 months) and shall not exceed $150,000 over the life of the BPA. ATF anticipates issuing multiple Blanket Purchase Agreements as a result of this notice. All offerors must submit their proposal response in accordance with the below documents, which are attached to this notice. Attachment 1 - Statement of Work Attachment 2 - Proposal Instructions Attachment 3 provides how ATF will evaluate the proposal. Attachment 4 is to be submitted for past performance/experience.
