Description
This is an INTENT TO MAKE A SINGLE SOURCE AWARD only. The Department of Veteran Affairs, Network Contracting Office (NCO) 21, intends to issue a single source purchase order to Pu'ulu Lapa'au. This single-source procurement is in accordance with FAR 13.106-1 (only one responsible source and no other supplies or services will satisfy agency requirements) for impaired physician/provider monitoring services. Services will be from 01/13/16- 09/30/2016 under NAICS 624190. Services include: CONTRACTOR PROVIDED SERVICE: A. Contractor shall assist Veterans Administration Pacific Islands Health Care System (VAPIHCS) in the review of health professionals deemed as having a possible impairment as defined in VAPIHCS Facility Policy Memorandum 11-13-009. B. Upon recommendation by VAPICHS, contractor shall communicate with the impaired provider in an effort to obtain voluntary entry into the monitoring program. If requested, contractor shall contact Regulated Industries Complaints Office and the appropriate state licensing board. Contractor shall notify VAPIHCS of any refusal of voluntary monitoring. C. Contractor shall refer impaired provider to appropriate independent healthcare professional or treatment program for initial evaluation, recommendations, and treatment. D. Contractor shall assist VAPIHCS in the review of health professionals deemed as displaying "disruptive behavior" as defined in VAPIHCS Facility Policy Memorandum 11-13-009 and potential enrollment in Behavioral Monitoring. E. Contractor shall, upon recommendation by VAPICHS that the health professional is a candidate for Behavioral Monitoring; contractor shall communicate with the "disruptive" provider in an effort to obtain voluntary entry into the behavioral monitoring program. If requested, contractor shall contact Regulated Industries Complaints Office and the appropriate state licensing board. Contractor shall notify VAPIHCS of any refusal of voluntary monitoring. F. Contractor shall refer "disruptive" provider to appropriate independent healthcare professional or treatment program for initial evaluation, recommendations, and treatment. G. Contractor shall maintain records relating to services provided in the form and manner as directed by VAPIHCS in accordance with VA policy. H. Contractor shall collaborate with VAPIHCS to develop a return-to-work agreement for the monitored healthcare professional. I. Contractor shall provide VAPIHCS two staff educational opportunities per contract year related to the health of healthcare professionals. J. Contractor shall provide services for all VAPIHCS Medical Staff and Allied Health Professionals, who are fully licensed or otherwise granted authority to practice in a State, Territory, or Commonwealth of the United States. Services include direct contact with impaired providers, weekly in-person visits with impaired providers, and working with healthcare providers in the State of Hawaii. This notice is neither a request for quotes nor a solicitation of offers; however, all responsible sources may submit a capability statement, proposal, or quotation, which shall be considered by the agency on future requirements. No solicitation package is available. A determination not to compete is solely within the discretion of the Government. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government and on an all or none basis. Interested parties must respond to this notice no later than 8:00 am on January 19, 2016. When responding to this announcement, respondents should refer to Announcement VA261-16-Q-0251 in the subject line and must utilize Microsoft Office 2010 software if responding via email for compatibility. Responding as an interested party under the www.fbo.gov website doesn't constitute convincing evidence of capabilities. All interested parties are reminded to be registered with System for Award Management (SAM) at https://www.sam.gov/portal/public/SAM/ in order to be eligible for award of Government contracts.
