- MTC Terms and Conditions W-9, Self-Certification, Vendor Set-Up DBA Wage Determination Construction Bid Package This is a subcontracting opportunity with Management and Training Corporation (MTC), operator of the Tongue Point Job Corps Center. Tongue Point Job Corps Center is requesting proposals from contractors providing services listed in the attached scope of work. Details of the solicitation are in the attached Construction Bid Package (CBP: TPJCC Install Water Heaters 16-02.pdf.) Please accept this as your invitation to attend our pre-bid meeting for this project at Tongue Point Job Corps Center, located at 37573 Old US Highway 30, Astoria, OR 97103. The meeting will be conducted on Wednesday January 27, 2016 at 1:00pm (PST), with bids due on Wednesday February 03, 2016 at 5:00pm (PST). Please see the attached Construction Bid Package (CBP) for details. Please pay close attention to page 2 of the Construction Bid Package Instructions to Bidder. In addition to the bid sheet, all valid bids must include 3 (three) references, DBA Worker Classification/Rates, a W-9, and completed MTC vendor self-certification form (attached in vendor packet). Bidders must self-certify under the NAICS code listed in the CBP. (NAICS codes can be found at http://www.census.gov/eos/www/naics/) This solicitation is listed under NAICS code: 238220: Plumbing, Heating and Air-Conditioning Contractors Please note: A site-inspection will be required by the bidder or their appointed company representative for the bid to be considered valid. The best opportunity for this inspection will be during the pre-bid meeting at the time stated above.
