PLEASE READ THE ENTIRE SOLICITATION: Offerors must complete and return all information as designated in 52.212-1, INSTRUCTIONS TO OFFERORS. Also comply with VHA Directives 6500.6, 6550 and MDs2 (attached) of the solicitation. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with commercial items procedures of FAR Part 12. Solicitation #RFQ VA256-16-R-0098 The solicitation number VA256-16-R-0098 is being issued as a Request for Quote (RFQ). This procurement request is for 'Semi-Annual Elevator Safety Inspection Services'. This procurement is set-aside 100% for Total Small Business concerns. The North American Industry Classification System (NAICS) code for this procurement is 541350; the business size standard is $7.5 million. The DUNS Number, the Small Business determination and tax identification numbers shall be submitted with quotes. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-85 effective December 04, 2015. INSTRUCTIONS TO OFFERORS An offer shall be considered complete if it includes the technical capability, price quote and the required documentation in accordance with Instructions to Offerors reference 52.212-3. As a minimum, offers must include- 1.A Technical Description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include QEI-1 licenses, Arkansas State permits and other relevant documents demonstrating capability. Complete 'Technical Capability' will be considered as (Attachment 1). 2.Offerors MUST return provision 52.212-3 with their proposal. McClellan Medical Center 4300 W. 7th Street, Little Rock, AR 72205 and Eugene J. Towbin Healthcare Center 2200 Ft. Roots Drive, North Little Rock, AR 72114 2.Scope of Work: The contractor shall provide a licensed QEI Inspector to perform safety inspections two times during 2016 and to witness only annual tests one time during 2016 on 18 units at McClellan Medical Center and on 25 units at Fort Roots Medical Center. The other option years inspections will be held biannually for the duration of the contract. 3.VAMC - LR/NLR will provide the Elevator Maintenance Company to accompany the QEI Inspector on each inspection. VAMC - LR/NLR will provide the Elevator Maintenance Company to perform the annual tests in the presence of the QEI Inspector. The Elevator Maintenance Company will provide all necessary tools and gauges to perform the annual tests. 4.Background: The following will describe more in-depth requirements pertaining to the repairs and service in which the contractor in question will perform on all passenger and service elevators located at John L. McClellan Medical Center and Eugene J. Towbin Healthcare Center. Inspections are to be made in accordance with ASME Al 7.2 2010 Guide for Inspection of Elevators, Escalators, and Moving Walks, which covers recommended inspection and testing procedures to conform to the ASME Al 7.1-2010 Safety Code for Elevators and Escalator, and ASME Al 7.3 2011 Safety Code for Existing Elevators and Escalators. Inspections will be thorough, complete, and technically sufficient. Vendor should perform duties with little interruption to customer and always clean up after work is completed. Vendor should always work within the guidelines of OSHA. At the completion of each inspection, the inspector will review his findings with the NLR Maintenance Engineer (COR) and obtain his signature. All deficiencies will be identified with the appropriate code violation. The original and two copies of the inspection forms along with a summary of the inspections will be presented to the NLR Maintenance Engineer (COR). 5.Performance Period: All work to be performed will be accomplished during normal business hours Monday through Friday. 6. Type of Contract: Firm-Fixed-Price. B.CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting, or has advised the contractor that a kick off meeting is waived. C.GENERAL REQUIREMENTS 1.For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, along with associated sub-milestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed work plan. 2.All written deliverables shall be phrased in layperson language. Statistical and other technical terminology shall not be used without providing a glossary of terms. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES. Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated above. Task One: The contractor shall provide a detailed work plan and briefing for the VA project team, which presents the contractor's plan for completing the work. The contractor's plan shall be responsive with this SOW and describe, in further detail, the approach to be used for each aspect of this work as defined in the technical proposal. E.SCHEDULE FOR DELIVERABLES 1.The contractor shall complete the Delivery Date column in Attachment A for each deliverable specified. 2.Unless otherwise specified, the number of draft copies and the number of final copies shall be the same. 3.If for any reason any deliverable cannot be delivered within the scheduled time frame, the contractor is required to explain why in writing to the CO, including a firm commitment of when the work shall be completed. This notice to the CO shall cite the reasons for the delay, and the impact on the overall project. The CO will then review the facts and issue a response, in accordance with applicable regulations. G.CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. H.REPORTING REQUIREMENTS 1.The progress report shall cover all work completed and shall present the work to be accomplished during the subsequent period. This report shall also identify any problems that arose, along with a statement explaining how the problem was resolved. This report shall also identify any problems that have arisen but have not been completely resolved, with an explanation. I.CONTRACTOR EXPERIENCE REQUIREMENTS (Standard mandatory) The government will determine on a SR basis what positions are considered key personnel. J.If a key person becomes unavailable to complete the SR, the proposed Substitutions of key personnel shall be made only as approved directed by the Contracting Officer and the COR. The government will not dictate specific experience and education requirements of the employees initially proposed to perform the work stated herein. The contractor shall submit a resume of qualifications for the COR and all other direct employees proposed for the project. If, at any time from date of award to the end of the contract, non-key personnel Contractor personnel are no longer available, HE/SHE reserves the right to review the qualifications of the proposed replacement personnel and to reject individuals who do not meet the qualifications set forth in the TLO. oThe contractor must inform the VHA COR, and Project Manager/Task Manager when personnel are removed from the contract for any reason. oIf a key person becomes unavailable to complete the SR, proposed Substitutions of key personnel shall be made only if approved by the COR and Project Manager. oThe contractor shall submit a resume of qualifications to the COR for all direct employees proposed for the project. Purchases paid with a Government-wide commercial purchase card are considered to be an electronic transaction for purposes of this rule, and therefore no additional electronic invoice submission is required. (c) Data transmission. A contractor must ensure that the data transmission method and format are through one of the following: (1) VA's Electronic Invoice Presentment and Payment System. (See Web site at http://www.fsc.va.gov/einvoice.asp.) (2) Any system that conforms to the X12 electronic data interchange (EDI) formats established by the Accredited Standards Center (ASC) and chartered by the American National Standards Institute (ANSI). The X12 EDI Web site (http://www.x12.org) includes additional information on EDI 810 and 811 formats. (d) Invoice requirements. Invoices shall comply with FAR 32.905. (e) Exceptions. If, based on one of the circumstances below, the contracting officer directs that payment requests be made by mail, the contractor shall submit payment requests by mail through the United States Postal Service to the designated agency office. Submission of payment requests by mail may be required for: (1) Awards made to foreign vendors for work performed outside the United States; (2) Classified contracts or purchases when electronic submission and processing of payment requests could compromise the safeguarding of classified or privacy information; (3) Contracts awarded by contracting officers in the conduct of emergency operations, such as responses to national emergencies; (4) Solicitations or contracts in which the designated agency office is a VA entity other than the VA Financial Services Center in Austin, Texas; or (5) Solicitations or contracts in which the VA designated agency office does not have electronic invoicing capability as described above. (End of Clause) C. 6 VAAR 852.237-70 CONTRACTOR RESPONSIBILITIES (APR 1984) The contractor shall obtain all necessary licenses and/or permits required to perform this work. He/she shall take all reasonable precautions necessary to protect persons and property from injury or damage during the performance of this contract. He/she shall be responsible for any injury to himself/herself, his/her employees, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by his/her employees fault or negligence, and shall maintain personal liability and property damage insurance having coverage for a limit as required by the laws of the State of. Further, it is agreed that any negligence of the Government, its officers, agents, servants and employees, shall not be the responsibility of the contractor hereunder with the regard to any claims, loss, damage, injury, and liability resulting there from.
