- Attachment 5 - Wage Determination Attachment 4 - Provisions and Clauses Attachment 3 - Evaluation Tables for LPTA Attachment 2 - Pricing Tables Attachment 1 - Statement of Work Request for Proposal Letter This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a separate written solicitation will not be issued. This notice is being issued as a Request for Proposals for Personnel Move Services in accordance with the attached Statement of Work. The solicitation number for this request is NNK16571085R. The provisions and clauses in the RFQ are those in effect through Federal Acquisition Circular 2005-85. This procurement is a total small business set-aside. The North American Industrial Classification System (NAICS) code for this acquisition is 238390, Other Building Finishing Contractors. The small business size standard for NAICS code 238390 is $15,000,000. The offeror shall state in their offer their size status for this procurement. The place of performance may include John F. Kennedy Space Center (KSC), Cape Canaveral Air Force Station (CCAFS), and/or Patrick Air Force Base (PAFB), Florida. In the event that supplies need to be delivered during the performance of this contract, delivery shall be FOB Destination to NASA KSC. Offers for the services described above are due by 2:00 PM ET on February 1, 2016 to NASA KSC and must include: 1. Solicitation number 2. FOB destination to KSC 3. Proposed schedule, discount/payment terms 4. Taxpayer identification number (TIN) 5. Identification of any special commercial terms, and 6. Signed by an authorized company representative 7. DUNS Number and CAGE Code 8. Other information as required by the attached Request for Proposal letter Offerors are encouraged to use the Standard Form 1449, Solicitation/Contract/Order for Commercial Items form found at URL: http://server-mpo.arc.na sa.gov/Services/NEFS/NEFSHome.tml The FAR and NASA FAR Supplement (NFS) provisions and clauses that apply to this acquisition are provided in Attachment 4. The provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (NOV 2015) is applicable to this acquisition. Offerors are required to return their completed representations and certifications with their proposal as identified in FAR 52.212-3.
