DISCLAIMER Sources Sought Only: This Request for Information (RFI) is issued solely for information and planning purposes only and does not constitute a solicitation; request for proposals or quotes; and does not obligate the Government to award a contract. In accordance with Part 10 of the Federal Acquisition Regulation (FAR), the U.S. Government is conducting market research. Responses to this Sources Sought must be in writing. Requests for a solicitation will not receive a response. All information received in response to this RFI that is marked as proprietary will be handled accordingly. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI; the Government will not pay for any information or administrative costs incurred in response to this sources sought. PURPOSE OF NOTICE This is a sources sought synopsis only. This is not a solicitation announcement. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (HubZone, 8(a), small, small disadvantaged or service disabled veteran owned small business) relative to NAICS Code 621399, Offices of All Other Miscellaneous Health Practitioners (size Standard $7.5 Million). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of responses to this sources sought synopsis, a solicitation announcement may be published on the FedBizOpps website. Responses to this sources sought synopsis are not considered adequate responses to a solicitation announcement. All interested offerors will have to respond to any subsequent solicitation announcement, if any is issued, in addition to responding to this sources sought announcement. The Network Contracting Office 17 (NCO 17) is seeking a potential qualified contractor to provide monitoring of permanent electrode placement for Veteran beneficiaries of the South Texas Veterans Health Care System - Audie L Murphy VA Hospital, located at 7400 Merton Minter San Antonio, TX 78229. The equipment provided must be brand name or equal. The Contractor shall provide the Neurophysiologist and necessary equipment, which shall be brand name or equal, capable of the following: 1.Equipment used to target placement area of implantation of the permanent deep brain stimulation electrode must be brand name or equal and have the following salient characteristics to advance or retract a 50-12-1 hydraulic probe drive to precisely position metal microelectrodes or pipettes: a.Multichannel microelectrode recording system b.Precision step motors and microdrive controller system c.Microdrive kit: microTargeting or Stardrive tray with stereotactic adapters i.motor controller module ii.motor coupling unit 2.Neurophysiologist with experience in MER and stimulation mapping techniques, and specialized training related to DBS mapping protocols and equipment. The Neurophysiologist must be physically present during implantation of electrodes and is responsible for to perform technical interpretation, microelectrode recordings, operating equipment, and consulting on procedure. The Government reserves the right to determine whether similar equipment presented by a contractor is equal to the equipment listed above with the required salient characteristics. INSTRUCTIONS: If you are interested and capable of providing the required services please provide the requested information as indicated below. The intended contract period will be for one base year plus four, one-year option periods. Responses to this notice should include the following: (1)General information about your company including: name, physical location/address, contact person, phone number, fax number, email address(s), DUNS, Tax ID number and company website if available. (2)Business Size pursuant to the following: a.Is your business small? b.If small, does your firm qualify as a small, emerging business or small disadvantaged business? c.If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? d.Is your firm a certified HubZone firm? e.Is your firm a woman owned or operated business? f.Is your firm a certified Service disabled Veteran Owned Small Business or Veteran Owned Small Business? (3)Verification that you possess all required equipment and personnel as identified by this notice. (4)How long has your company existed? How many years of experience does the neurophysiologist have? If a solicitation is issued in the future, all interested parties must also respond to that solicitation announcement separately from the responses to this announcement for Sources Sought. Responses to this sources sought is not equivalent to a request to be added to a prospective bidders list or to receive a copy of the solicitation. Not responding to this sources sought does not preclude participation in any future solicitation, if any is issued.
