Response date (Deadline): January 27, 2015 at 1:00pm Statement of Work (SOW) A.GENERAL INFORMATION 1.Title of Project: Removal of radioactive cesium sources from obsolete liquid scintillation counters on the Research Service 2.Background: The Research Service has one obsolete liquid scintillation counter and would like to dispose of these instruments. Prior to doing so the instrument must be decommissioned by removal and proper disposal of radioactive source(s) contained in the machine and transfer of those. This work requires highly trained technicians knowledgeable about the specific pieces of equipment and where in the instrument the radioactive source(s) are housed. Work conducted with radioactive materials falls under the purview of specific federal regulations including those of the US Nuclear Regulatory Commission. To assure that the work is properly conducted, our local radiation safety officer has determined that technicians from the manufacturer must perform the work. 3.Scope of Work: Removal of radioactive sources from Rackbeta 1214 instrument. 4.Period of Performance: 30 days. 5.Type of Contract: Firm Fixed Price 6.Place of Performance: The Contractor shall support this effort at the Department of Veterans Affairs Medical Center, 130 West Kingsbridge Road, Bronx, NY 10468. The COR will provide adequate on-site workspace and automated data processing (ADP) equipment for Contractor personnel. Work at the government site shall not take place on Federal holidays or weekends unless directed by the Contracting Officer (CO). B.GENERAL REQUIREMENTS 1.The contractor will conduct the work specified above in coordination with JJPVA Research Office and Radiation Safety Office. The contractor will have the personnel, equipment, material, supplies, forms, and systems of computer programs to conduct this work, unless otherwise stated in a specific contract. C.SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Task 1: Select Personnel Schedule a visit by a technician trained in removal of radioactive sources from PERKINELMER liquid scintillation counters. Task 2: Regular Communication/Meetings and Recording Decisions Remove all radioactive sources from each of the Rackbeta liquid scintillation counter presently on-site in the Research building at JJPVA. Task 3: Project Plan and Procedures Removal of radioactive sources from the instrument and disposal of these sources in compliance of the NRC and the JJPVA radiation safety officer. D.SCHEDULE OF DELIVERABLES Removal and disposal of radioactive: 30 days. E. INSPECTION AND ACCEPTANCE CRITERIA Final inspection and acceptance of all work performed, reports and deliverables will occur at the place of delivery designated by the COR. Final approval and acceptance of documentation required herein shall be by letter of approval and acceptance by COR. The Contractor shall not construe any letter of acknowledgment of receipt of material as a waiver of review, or as an acknowledgment that the material is in conformance with this SOW. F. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. G. GOVERNMENT RESPONSIBILITIES The government shall assure that the contractor has access to VA toll-free telephone numbers for Veterans as appropriate. H. KEY PERSONNEL The Contractor shall be responsible for managing and overseeing the activities of all Contractor personnel, as well as subcontractor efforts used in performance of this effort. The Contractor's management responsibilities shall include all activities necessary to ensure the accomplishment of timely and effective support, performed in accordance with the requirements contained in the statement of work. The Contracting Officer may notify the Contractor and request immediate removal of any personnel assigned to the task order by the Contractor that are deemed to have a conflict of interest with the government or if the performance is deemed to be unsatisfactory. The reason for removal will be documented and replacement personnel shall be identified within three business days of the notification. Employment and staffing difficulties shall not be justification for failure to meet established schedules. Type of Procurement: The responses to this Request for Information (RFI) will help determine what, if any, type of set-aside. Contract Vehicle: The resulting contract vehicles from this process will be a firm fixed price. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities which you have provided services to, DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business). If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in Central Contractor Registry (CCR) (see internet site: http://www.ccr.gov). Responses to this notice must be submitted in writing (email) and received not later than January 27, 2015 at 1:00PM.
