Description

This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA248-16-Q-0458 is issued as a (Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-85-1/01-04-2016. Only emailed requests received directly from the requester are acceptable. Responses should be sent to christopher.brailer@va.gov. No telephone responses will be accepted. To be receiving a government award, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. Online Representation and Certification Application (ORCA) must also be current to qualify to receive a contract award. Offeror may register with CCR at www.orca.bpn.gov NAICS and Set-aside: The NAICS is 811219 and the small business size standard is $20.5 million. Any firm that is not considered a small business under this NAICS code should not submit a response to this notice. Description of Services: The VHA NCO 08 - Malcom Randall VAMC, ENT clinic, Gainesville, FL requires the following services: Scope of Work: A field engineer technically trained on the Stryker powered surgical instruments will come on-site twice a year to inspect an run diagnostics on all covered equipment, any equipment found during this time or suspected at other times to be functioning improperly will be repaired or replaced as necessary under this contract. Overnight loaners will be provided for any equipment shipped off-site for repair. Covered Equipment: SYSTEM 6 SAG SAW QTY:7 SYSTEM 6 DUAL TRIGGER QTY:7 SYSTEM 7 RECIP SAW PROCARE QTY:7 SYSTEM 6 PRECISION QTY:4 SYSTEM 6 ATTACHMENTS QTY:35 CORDLESS DRIVER 3s QTY:7 CORDLESS/ROTARY ATTACHMENT QTY:35 SYSTEM 6 BATTERIES QTY:24 SYSTEM 6 BATTERY CHARGERS QTY:3 FLYTE & T5 CHARGERS QTY:3 FLYTE LIGHTED HELMETS QTY:4 FLYTE HELMETS QTY:4 T5 SURGICAL HELMETS QTY:2 CORE CONSOLES QTY:2 CORE IMPACTION DRILLS QTY:10 TPS HANPIECE CORDS QTY:10 TPS FOOTSWITCHES QTY:2 Data/Deliverables 1)Two preventative maintenance per unit per year 2)Complimentary replacement or loaners with overnight delivery 3)Unlimited repairs and service calls 4)All emergency calls are included as well as all travel, labor and parts. 5)Any safety modification to the equipment when necessary at no charge to the customer Scheduling and Hours of Work 1)All work should be scheduled with the VA Operating Room Supervisor or biomedical engineering technician at 352-548-6000 x2435 or x2436 2)Work shall be scheduled for proper execution to completion of this contract. 3)Field engineer onsite coverage will be Mon-Fri 8:00am-5pm, if afterhours is needed, FSE must coordinate with Biomedical Engineering Technician or Supervisor. 4)Written report of all findings and recommendations provided to customer within five business days. Contract Type: The government anticipates awarding a Firm Fixed Price award for a base year and 4 option years. Award will be made to lowest price quote which conforms to the requirements within this solicitation. Submitting Quote: Contractor shall submit their quote on company letterhead and shall include unit price, unit quantity, item description as specified above and, total price; any offered discounts; proposed delivery time; name, address, and telephone number of the offeror; terms of any express warranty; completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and ORCA document. Quotes must be received February 5, 2016 by 1300 EST. All quotes must be submitted by electronic submission in reference to VA249-16-Q-0458 to christopher.brailer@va.gov; Offers received after the exact time specified in the solicitation are considered late and may be considered if the action would not unduly delay the acquisition. All questions regarding this solicitation must be submitted to the Contracting Specialist in writing (email is acceptable).
