Sources Sought Notice (SSN) Experience Template This is a Sources Sought Notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The National Labor Relations Board (NLRB) is issuing this notice to obtain information to assist in determining the appropriate acquisition method to procure Ergonomic Consultation and Evaluation Services for the Reasonable Accommodation Program (RAP). The anticipated period of performance for this requirement is five (5) years; a 12-month base period, and four (4) 12-month option periods. Service is anticipated to commence in April 2016. The following section specifies the Performance Elements for the requirement: a. Ergonomic Evaluations: The contractor shall provide ergonomic consultation and evaluation services for NLRB employees. Evaluations are conducted following a health care provider referral or RA request approved by the Reasonable Accommodation Program Manager (RAPM) and are based on a documented medical condition. The contractor shall provide a thorough assessment of the employee's work environment. The evaluation may result in immediate recommendations, proposed work environment modifications, and/or equipment recommendations. Equipment recommendations shall take into consideration the employees existing working environment to ensure recommended equipment will be compatible. Prior to submitting the detailed written report to the COR, the contractor shall discuss recommended equipment with the employee. b. Ergonomic Adjustments: In the event that a follow-up adjustment is required, the contractor shall review the installed equipment and modifications made to determine if the changes are sufficient and correct, to demonstrate correct usage of the ergonomic modifications, and to make any additional adjustments and/or recommendations necessary. To this end, NLRB is seeking the following information from interested companies: 1) A demonstrated understanding of the requirement, with specific demonstrated experience in the following areas: a. Ability to provide ergonomic consultation and evaluation services throughout the United States. The NLRB has regional offices in cities across the US and Puerto Rico. b. Experience successfully staffing highly qualified Certified Ergonomic Professionals. c. Relevant Experience: Provide information that demonstrates the extent of your experience performing the same or similar work during the last three (3) years [See Attachment #1 -Experience Template]. NOTE: Prospective contractors shall be registered in the System for Award Management (SAM.gov) database prior to award, during performance, and through final payment. INFORMATION SUBMISSION INSTRUCTIONS: The response to this notice is limited to 2 pages, excluding attachments (single spaced, 11 point font minimum) and should also include current business size status, and any applicable GSA Federal Supply Schedules. This indication should be clearly marked on the first page of the response, as well as the eligible business concern's name, point of contact, address, and cage code. All responses to this Sources Sought Notice must be submitted electronically (via e-mail) to Julie Yarbrough, Contract Specialist, at julie.yarbrough@nlrb.gov CUT-OFF TIME/DATE: Capability statements with respect to the information above are due no later than 10:00 AM (ET) on FEB 3, 2016. Capability statements received after this date and time may not be considered. This notice does not obligate the Government to award a contract or otherwise pay for the information provided in the response. Any concern responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the concern's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation MAY be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. The Government will NOT accept requests for meetings or briefings. No phone solicitations or email with regards to the status of the Request for Proposal (RFP) will be accepted prior to its release. Information and materials submitted in response to this notice WILL NOT be returned. The Government does not intend to award a contract on the basis of responses received nor otherwise pay for the preparation of any information submitted. As a result of this notice, the Government MAY issue a Request for Proposal (RFP). There is no solicitation available at this time. However, should such a requirement materialize, no basis for claims against the Government shall arise as a result of a response to this Sources Sought Notice or the Government's use of such information as either part of our evaluation process or in developing specifications for any subsequent requirement. CONFIDENTIALITY: No proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s).
